高压氧对HIE新生儿细胞免疫功能的影响及其机制的研究

摘要 目的观察高压氧(HBO)对缺氧缺血性脑病(HIE)新生儿T细胞亚群的影响,并探讨β-内啡肽(β-EP)所起的作用。方法随机选取治疗组和对照组,进行治疗组在1疗程HBO治疗前、后各1次及对照组同期1次T细胞亚群及β-内啡肽的检测。结果HBO治疗前HIE患儿CD3、CD4、CD8、CD4/CD8相比对照组无显著性差异(P>0.05);HBO治疗后对比HBO治疗前CD3、CD4、CD4/CD8均明显下降,而β-EP、CD8则显著上升(P<0.05)。β-EP与T细胞亚群HBO治疗前后差值的相关分析示:除CD8外,β-EP的差值与各项指标的差值均有显著的相关性,相关系数在0.325~0.635之间。结论HBO对HIE患儿细胞免疫功能有明显抑制作用,而β-EP水平的升高是其免疫下调的机制之一。 Objective To explore the effection about the hyperbaric oxygenation to the T-cell subpopulation of hypoxic-ischemic encephalopathy newborns and the pathophysiologic of β-endorphin (β -EP). Methods Randomly choose treat group and control group. T-cell subpopulation and β - endorphin was detected in treat group once time before and after a course of hyperbaric oxygenation treatment and homochrome detected once in control group. Results Before hyperbaric oxygenation treatment, HIE infants' CD3, CD4, CD8, CD4/CD8 all decent a little contrasted with control group, but no obvious difference (P〉0. 05);After hyperbaric oxygenation treatment ,HIE infants' CD3 CD4 CD4/CD8 decent obviously, but only CD8 ascend obviously (P〈0. 05). relative analysis the balance of β-EP and the T-cell subpopulation before and after HBO treatment showed: the correlation between the balance of β-EP and the balance of each index except CD8 were obvious, correlation coefficient (r) is between 0.325~0.635. Conclusion Hyperbaric oxygenation have obvious inhibition to cellular immune function of newborns with HIE, and the acent of β-EP is one of the mechanisms of immunologic function descent.
