摘要
目的探讨修订Tokuhashi评分对脊柱转移瘤的临床应用价值.方法1999年7月～2004年3月,应用修订Tokuhashi评分对28例脊柱转移瘤进行评估,根据评分结果选择治疗方法;其中21例Tokuhashi评分≥5分者行手术治疗.结果随访7个月～5年,Tokuhashi评分＜5者,平均生存期2.3个月(1.0～3.5个月),Tokuhashi评分5～8分者,平均生存期5.3个月(3～7个月),Tokuhashi评分9～12分者,平均生存期16.5个月(5.5～58个月).修订Tokuhashi评分总体预测准确率为78.6%.结论修订Tokuhashi评分有助于指导生存期预测及治疗方式选择.
Objective To assess the clinical value of the modified Tokuhashi rating system in evaluating spinal metastases patients. Method From Jul 1999to Mar 2004, twenty - eight patients with spinal metastases were studied. The treatment for metastatic spine tumors patients had been prospectively decided based on modified Tohuhashi system. Ten patients were with the score 9 to 12 and underwent aggressive curettage or total piecemeal resection of tumors with reconstruction and stabilization by posterior approach. Eleven patients with the score 5 to 8 and underwent palliative posterior decompression and stabilization. Seven patients were with score of O to 4 and received non - operative and supportive care. Results The follow - up ranged from 7 months to 5 years . The mean survival of the 10 patients with a total score of 9 to 12 was 16.5 months （5.5 -58 months） and the ll patients with a total score of 5to 8 was 5.3 months （ 3 - 7months） and the 7 patient whose total score was less 5 was 2.2 months （ 1 - 3.5months）. The sum accuracy of the modified Tokuhashi score for predicted prognosis was 78.6%. Conclusion The modified Tokuhashi rating system is reliable for estimating life expectancy and it is useful in making decision regarding treatment.
