摘要 目的:研究人工全髋关节置换术(THR)后假体周围的骨质丢失及假体松动的防治方法。方法:将39例进行骨水泥型Ⅰ期全髋关节置换的患者,分成2组,观察组22例,术后每日服用10mg阿仑膦酸钠+钙剂(50mg),对照组17例,术后服用50mg钙剂。随访12个月,测定假体周围骨矿密度(BMD)。结果:观察组在相同随访时间里,假体周围相同区域BMD降低不明显或升高。在股骨近端(prROI),对照组的BMD平均降低(21.2±5.2)%,而观察组平均升高(4.2±2.6)%,2组比较差别有统计学意义(t=5.995,P<0.05),在随访期末2组间全假体周围(totROI)平均BMD改变,对照组下降(12.1±6.2)%,而观察组升高(2.6±1.4)%,差别有统计学意义(t=6.413,P<0.05)。结论:阿仑膦酸钠可明显降低骨水泥型Ⅰ期全髋置换术后骨质丢失,改善骨骼对假体的支撑作用,可能会预防或延缓假体松动。 Objective: To study the prevention and treatment method of periprosthetic bone loss after cemented total hip replacement (THR). Methods: Thirty-nine cemented primary THR patients were randomly divided into two groups including 22 in observing group with 10 mg alendronate plus 50 mg calcium and 17 in control group with 50 mg calcium. All patients were followed up for 12 months, and the pcriprosthetic bone mineral density (BMD) was measured. Results: The periprosthetic BMD decreased insignificantly or increased in observing group in the same follow-up. The BMD of the proximal femur (prROI)meanly decreased (21.2 + 5.2) % in control group and increased (4.2 + 2.6) % in observing group (t = 5.995, P〈 0.05), The mean periprosthetic BMD of the total pcriprosthetic area (totROI) decreased in control group (12.1 + 6.2) % and increased in observing group (2.6 + 1.4) % at the end of follow-up. There was a significant difference between the two groups (t = 6.413, P 〈0.05). Conclusion: Alcndronatc significantly decreases periprosthetic bone loss after primary cemented THR, improves the support of the prosthesis, and prevents the prosthesis flexible.
