摘要
目的观察幽门螺杆菌(Hp)感染及血清白细胞介素8(IL-8)与初发脑梗死的关系,探讨IL-8在Hp感染可能导致脑梗死机制中的作用。方法80例初发脑梗死(观察组),其中动脉粥样硬化性血栓性脑梗死49例(A组),腔隙性脑梗死31例(B组)。采用病例对照1∶1配对的研究方式选择对照,检测观察组和对照组血清标本的Hp特异性抗体IgG(Hp-IgG)、IL-8、C-反应蛋白(CRP)浓度等。结果观察组Hp-IgG阳性率为81%,对照组为58%,二者比较差异有统计学意义(P<0.01);A组Hp-IgG阳性率为88%,对照组为55%,二者比较差异有统计学意义(P<0.01);B组Hp-IgG阳性率为71%,对照组为61%,二者比较差异无统计学意义(P=0.41)。观察组的IL-8浓度高于对照组(P<0.01),IL-8浓度与Hp-IgG浓度、与CRP浓度、临床神经功能缺损程度分数正相关(P值均<0.01)。结论Hp感染与动脉粥样硬化性脑梗死发病相关;脑梗死发病后血清IL-8浓度升高,并且与Hp-IgG浓度、CRP浓度、临床神经功能缺损程度评分正相关。
Objective To observe the relationship among Helicobacter pylori(Hp),interleukin-8(IL-8) and primary cerebral infarction(CI) and to study the possible effect of IL-8 on cerebral infarction in Hp infection. Methods 80 cases with primary cerebral infarction (observe group)were studied. Among them,atherosclerosis thrombus cerebral infarction were found in 49 cases (group A), lacunar infarct were found in 31 cases (group B). The control group was choiced according to 1:1 paired case control study. The Hp specificity immune body IgG (Hp-IgG) density, IL-8 density, C-reactive protein (CRP) density in blood serum specimen of the observe group and the control group were examined. Results The Hp positive infection rate was 81% in the observe group, that of the control group was 58%, There was significant difference between the two groups(P<0.01). The Hp positive infection rate of group A was 88%, that of the control group was 55%, There was significant difference between the two groups (P<0.01). The Hp positive infection rate of group B was 71%, that of the control group was 61%, there was no significance in the two groups (P=0.41). The observe group's IL-8 density was higher than that of the control group (P<0.01). There showed a relationship among IL-8 density, Hp-IgG density (P<0.01), CRP density (P<0.01) and clinical nerve function damage degree score (P<0.01). Conclusion The Hp infection has relation with atherosclerosis cerebral infarction. After cerebral infarction, the IL-8 density is elevated and have positive correlation with the Hp-IgG density, the CRP density and the clinical nerve function damage degree.
