危重病患者周围动脉和中心静脉血气比较及其评价 被引量:2

Evaluating and Comparing between Peripheral Arterial and Central Venous Blood Gas in Critical Care Patients

摘要 52例危重病患者周围动脉和中心静脉血气酸碱平衡状态进行比较,提示正常心功能和轻度心衰患者,动静脉血气酸碱平衡状态无明显差异(P>0.05)。重度心衰和循环衰竭患者,动静脉血气酸碱平衡状态出现明显分离现象。动脉血示低碳酸血症,而静脉血则反之,两者有明显差异(P<0.05)。结果表明,重度心衰和周围循环衰竭患者,单纯监测动脉血血气,并不能反映组织内确切的酸碱状态,周围动脉血和中心静脉血必须同时进行监测。在严重低心排出量和低灌流病人,监测中心静脉血能较好地反映组织酸碱状态。 Acid-base status between peripheral arterial and central venous blood gas analysiswere compared in 52 patients. It indicated that the acid-base status between peripheral arterial and central venous blood gas had no significant difference in patients with normalcardiac function or with mild cardiac failure, but there was a marked paradox between pesipheral arterial and central venous blood gas in the patients with severe cardiac and circulatory failure. Arterial blood showed hypocapnia but not the venous blood (P<0. 05).We concluded that the samples of central venous blood in addition to the arterial bloodshouId be required for the appropriate monitoring the acid-base composition in patientswith cardiac and circulatory failure- The central venous blood gas is superior to the arterialblood gas in the assessment of the acid-base status in these patients.
