摘要
目的:加深对宫颈癌多层螺旋CT表现的认识,探讨宫颈癌的多层螺旋CT诊断要点、分期及临床应用价值。方法:回顾性分析56例经手术病理证实或病理活检证实的宫颈癌多层螺旋CT表现及放疗后改变。结果:其中鳞癌49例、腺癌7例。Ib～Ⅱa期患者29例,行子宫全切除加盆腔淋巴结清扫;Ⅱb期患者21例,行介入局部动脉化疗后,再行手术根治;Ⅲ期～Ⅳ期患者6例,未手术、行放疗加化疗。结论:多层螺旋CT能显示宫颈和宫体的动脉期及静脉期强化时相,改善血管强化效果,利于癌肿血供情况的观察和显示癌肿的实际范围;获得连续的薄层图像;能从横断面、矢状面及冠状面显示宫颈形态改变,辨别宫旁结构,阴道受侵及盆腔淋巴结、盆壁转移征象,对宫颈癌的分期准确,了解术后有无复发及放疗效果。但对部分Ia期病例无法显示宫颈癌确实部位,早期宫颈癌仍需结合病理组织活检确诊。
Objective Strengthens the understanding to the cervical carcinoma that is displved by the spiral CT,probes into the main point of cervical carcinoma diagnosing,by stages and clinical using value.Methods Analyses 56 patients with cervical carcinoma confirmed through pathology verification and pathology changed after the radiotherapy by the spiral CT.Results Among them,there are 49 cases of squama cancer and 7 cases of adenocarcinoma.There are 29 patients with Ib～Ⅱa cancer to have been performed the operation of uterus excising and pelvis lymph clear-out;there are 21 patients with Ⅱb cancer had been performed the oper ation that effects a radical cure after getting involved in the local artery chemotherapy;there are 6 datients with Ⅲ～Ⅳ cervical carcinoma who had been performed the radiotherapy and chemotherapy without the operation.Conclusion The spiral CT can display the time phase strengthening of the artery and vein of uterus neck and its body, improve the blood vessel and treatment effects so that it is easy to observe the blood supplying of the tumor and display its actual range;obtain the continuous thin layer picture;Can display the changing form of the uterus neck through the cross section,arrow and coronary form,and distinguish the structure around the uterus and the symptom of the vagina infected,pelvis lymph gland and pelvis surface metastasis to confirm the stage of cervical carcinoma and find out whether the cervical carcinoma is recurred after operation and the effect is good or not after the chemotherapy.But to some cases with Ia cervical carcinoma,the actual part of cervical carcinoma can not be displayed,so the nonage cervical carcinoma still need the living examination to pathology organ.
