摘要
目的 探讨高龄肺癌患者的围手术期的处理。方法 总结64 例70 岁以上老年肺癌患者围手术期的临床经验。结果 术后并发心律失常9例,心功能不全2例,呼吸功能衰竭14例,死亡2 例,死亡率2.6%。结论 ①术后常见并发症为呼吸系统并发症和心血管系统并发症;②常见死亡原因为呼吸功能不全和心功能不全;③围手术期预防及处理:术前应注意呼吸道的准备。术后有效保持呼吸道通畅,必要时行呼吸机辅助呼吸。对术前有冠心病、肺心病及其它严重伴发疾病者应积极治疗,术后应有效地止痛、镇静,严密做床旁心电监测,及时分析心律失常的原因并做相应处理,注意保护心脏。
Objective To analyze perioperative treatment of thoracic surgery in elderly lung cancer patients.Methods The clinical experience of preoperation,perioperation and postoperation of 64 patients over 70 years old were studied.Results Postoperative complication occurred,such as 31 cases of arrhythmia,2 cases of heart failures,14 cases of pulmonary insufficiency .Two died.Conclusion ①Respiratory and cardiovascular systems complication are the common in patients with primary lung cancer.②The major causes of perioperative death in lung cancer patients are pulmonary and cardiac insufficiency.③Elderly patients with lung cancer hadve a decreased organ function and body resistance,and in most cases.It concurrs with many other chronic disease.These patients has poor tolerability to the chest-open surgery,and bear high risks of developing complication.There should be emphasis on the perioperative management.
