摘要
目的:就几种传统及新型抗癫痫药造成骨损害的致病机制、临床表现及检测指标等方面进行综述。资料来源:应用计算机检索Medline1992-01/2003-01关于儿童骨损害与抗癫痫药的文章,检索词“antiepilepticdrug,anticonvulsant,bonedensi-ty,bonemetabolism,并限定文章语言种类为English。同时计算机检索中国期刊全文数据库1994-01/2004-07关于儿童骨损害与抗癫痫药的文章,限定文章语言种类为中文,检索词“抗癫痫药,骨密度,骨代谢”。资料选择:对资料进行初审,选取实验包括实验组和对照组的文献,筛除明显不随机实验的研究,对剩余的文献开始查找全文,进一步判断为随机对照实验。纳入标准为①随机对照实验,采用单盲,双盲或非盲法。②实验包含平行对照组。③治疗组为使用抗癫痫药组。排除标准:重复性研究。资料提炼:共收集到25篇关于抗癫痫药致骨骼损害的随机和未随机实验文章,19个实验纳入标准,另有5篇为理论探讨。排除的1篇实验为重复的同一研究。资料综合:19个实验包括骨密度、骨代谢生化指标、骨矿物质等多项检测方法,并对其意义进行评价。结论:已证实抗癫痫药对骨骼发育有一定的影响,癫痫患儿在服用抗癫痫药的同时,要采用补充维生素D及钙治疗。
OBJECTIVE: To review the pathogenesis, clinical manifestations and detecting indexes of bone damage induced by several traditional and new type antiepileptic drugs.DATA SOURCES: An online search of Medline database was undertaken by using the keywords of “ antiepileptic drug, anticonvulsant, bone density, bone metabolism' to identify the relevant articles about bone damage and antiepileptic drug in children published in English from January 1992 to January 2003. Meanwhile, Chinese journals full text database was scanned with computer to search relevant articles about bone damage and antiepileptic drug in children published from January 1994 to July 2004, the key words were “ antiepileptic drug, anticonvulsant, bone density, bone metabolism' with language limited to Chinese.STUDY SELECTION: After the above search, the trials including treatment and control groups were selected, while the obvious non randomized trials were excluded, and then the full texts of the selected articles were searched for randomized controlled study. Inclusion criteria: ① randomized controlled study with single blind, double blind or non blind method; ② experiments including parallel control groups. Exclusion criteria: repetitive study.DATA EXTRACTION: Twenty five randomized and non randomized experimental articles about the bone damage caused by antiepileptic drugs were collected,19 of them were in accordance with the inclusion criteria, 5 were theoretical explorations, and the other 1 being excluded was repetitive study.DATA SYNTHESIS: The 19 experiments included many detecting methods of bone mineral density (BMD), biochemical indexes of bone metabolism and bone mineral matter, and the significance was evaluated.CONCLUSION: It has been confirmed that antiepileptic drugs can affect the development of skeleton. Children with epilepsia, who take antiepileptic drugs, should have vitmine D and calcium as supplements at the same time.
