摘要
目的介绍应用交锁髓内钉固定治疗前臂尺桡骨粉碎性骨折的经验。方法对2 1例尺桡骨骨折,按AO分型[1] B3 型8例、C1型10例、C2 型3例,采用骨折复位、Smith Nephew交锁髓内钉固定术。结果术后随访6~3 6个月,骨折愈合时间8~2 0周,平均16.2周。术后1例骨折延迟愈合,1例锁钉钉道感染,拔钉后愈合,无1例发生骨不连。按Anderson评分标准评定优10例,良7例,可4例。肘关节伸屈活动功能丧失10°~3 5°,平均18.5°,前臂旋前功能丧失15°~5 5°,平均2 5 .0°,旋后功能丧失15°~45°,平均3 3 .5°。结论应用交锁髓内钉固定方法是治疗前臂软组织损伤同时伴有尺桡骨粉碎性骨折的良好手术选择。
Objective To discuss the feasibility and effectiveness of interlocking intramedullary nailing for the treatment of comminuted forearm fractures. Methods 21 cases of forearm both bone fractures accompanied with soft tissue injuries were invovled in the study. According to AO classification, there were 8 cases of type B_3, 10 cases of type C_1, and 3 cases of type C_2. The radial and ulnar fractures were reduced and fixed by Smith-Nephew interlocking intramedullary nailing system. Postoperatively the patients were followed for 6 to 36 months. Results Bone healing was observed on average 16.2 weeks postoperatively, ranging from 8 weeks to 20 weeks. There was 1 case of delayed union, and 1 case of nail track infection which was solved after nail removal. None had non-union. The average loss of elbow motion, forearm pronation and supination were 18.5°, 25.0°, and 33.5° respectively. According to Anderson's evaluation standard, there were 10 cases graded as excellent, 7 cases as good, and 4 cases as fair. Conclusion Interlocking intramedullary nailing is a preferable method for treatment of comminuted forearm fractures accompanied with soft tissue injuries.
