腹腔镜结直肠癌根治术围术期免疫功能变化 被引量:5

Changes of systemic immune function in patients undergoing laparoscopy-assisted colorectal tumor surgery

摘要 目的探讨腹腔镜结直肠癌根治术围术期系统免疫功能的变化。方法22例拟行结直肠癌根治术的患者分为两组,分别为腹腔镜组(L组)和传统开腹组(O组),每组11例。分别于术前、术后24、48、72h抽取患者外周静脉血检测Th1、Th2细胞含量、Th1/Th2及IL-2、IL-6含量,检测术前、术后24h中性粒细胞吞噬功能。结果术后24h,L组Th1细胞含量高于O组(P<0.05);术后24、48、72h,L组Th2细胞含量低于O组,Th1/Th2高于O组(P<0.05);术后48h,L组IL-2含量高于O组,术后24hL组IL-6含量低于O组(P<0.05)。两组患者术后吞噬细胞百分数、吞噬指数较术前均下降(P<0.05),组间比较无统计学差异(P<0.05)。结论腹腔镜结直肠癌根治术对患者术后系统免疫功能的抑制有一定的保护作用,可能对患者的预后有益。 To investigate the changes of systemic immune function in patients undergoing laparoscopy-assisted colorectal tumor surgery during peri-operation period. 22 patients with colorectal tumor undergoing elective surgery were assighed to group L (laparoscopy-assisted group) and group O(conventional open abdominal group),with 11cases in each group. Peripheral venous blood samples were taken to determine the level of Th1, Th2,Th1/ Th2, IL-2 IL-6 at preoperation and postoperation 24, 48, 72 h, respectively. Phagocytic function of neutrophil were also observed at preoperation and 48 h postoperation. The level of Th1 in group L was higher than that of group O at 28 h postoperation (P<0.05). The level of Th2 in group L was lower than that of group O, while Th1/Th2 higher than that of group O at 24, 48, 72 h postoperation (P<0.05).Compared with group O, The level of IL-2 was higher at 48 h postoperation and the level of IL-6 was lower at 24 h postoperation in group L. The percentage of phagocytic cell and phagocytic index in both groups were decreased remarkablely after operation, but there were no significant difference between the two groups (P>0.05). [Conclusions] The systemic immune function in patients with laparoctomy-assisted colorectal cancer surgery can be partly preserved during postoperative period, which may be benificial to outcome of the patients.
