摘要
目的了解慢性闭角型青光眼的荧光血管造影表现和视盘血供状态.方法用海德堡视网膜造影仪(HRA)对慢性闭角型青光眼(chronic angle closure glaucoma,CACG)17例25只眼、原发性开角型青光眼(primary open angle glaucoma,POAG)18例32只眼和正常眼压性青光眼(normal tension glaucoma,NTG)14例27只眼进行眼底荧光血管造影,分析了视盘影像特点,并对视盘荧光充盈缺损和渗漏以及相关因素进行了统计比较.结果 CACG视盘荧光充盈缺损发生率为50%(11/22),POAG为56.7%(17/30),NTG为60%(15/25),差异无显著性意义(χ2=0.488,P=0.783).CACG视盘渗漏发生率为20.8%(5/23),POAG和NTG为15.4%(8/52),差异无显著性意义(χ2=0.344,P=0.558).视盘荧光充盈缺损(+)组的MD为(11.52±7.35)dB,(-)组为(8.27±7.03)dB,差异有显著性意义(Z=-2.007,P=0.045).视盘荧光充盈缺损(+)组以及视盘渗漏(+)组的年龄均大于(-)组(t=2.379和2.103,P=0.02和0.04).视盘荧光充盈缺损和视盘渗漏均以男性多见(χ2=4.11,P=0.042,P=0.0008).结论 CACG与POAG、NTG一样,视盘FFA有荧光充盈缺损与渗漏现象,说明同样存在视盘缺血的状态,其发生机制推测是压力因素引起视神经病变后的继发性改变.
Objective To evaluate the blood supply of the optic disk in patients with chronic angle closure glaucoma.Methods Using Heidelberg Retinal Angiography,we performed fundus fluorescent angiography(FFA)in 25 eyes of 17 chronic angle closure glaucoma(CACG)subjects,32 eyes of 18 primary open angle glaucoma(POAG)subjects and 27 eyes of 14 normal tension glaucoma subjects.All FFA results of optic disk were evaluated.The relating factors with the fluorescent defect and leakage were analysesed.Results The frequency of fluorescent defect of optic disk was 50%(11/22)in CACG,56 7%(17/30)in POAG,60%(15/25)in NTG eyes.There was no significant difference(χ 2=0 488, P =0 783).The frequency of fluorescent leakage of optic disk was 20 8%(5/23)in CACG,15 4%(8/52)in POAG and NTG eyes.There was no significant difference(χ 2=0 344, P =0 558).MD(octopus 101)of eyes with fluorescent leakage of optic disk were significantly bigger than those without it(Z=-2 007, P =0 045).The subjects with fluorescent defect or leakage of optic disk were elder than those without it(t=2 379 and 2 103, P =0 02 and 0 04).There were more male subjects with fluorescent defect or leakage of optic disk than females(χ 2=4 11, P =0 042, P =0 0008).Conclusions As POAG and NTG,there are also blood supply disorders in optic disk of CACG eyes,which might be the second result of glaucomatous neuropathy because of the raised IOP.
