摘要
目的探讨、研究原发性肝癌自发性破裂的手术切除指征。方法回顾性总结了1993年1月～2003年8月施行肝癌切除术的16例自发性破裂肝癌病例,并以同期随机抽取16例非破裂肝癌病例作为对照,综合比较、分析两组患者术前、中、后的临床资料。结果破裂与非破裂组术前肝功能 Child-Pugh 分级、肿瘤包膜及门静脉浸润,术中肝门阻断时间、术时及住院时间差异无显著性。破裂组肝脏肿瘤直径、术中失血量、术后并发症发生率和住院病死率均较非破裂组显著高。结论原发性肝癌自发性破裂患者是否一期手术在于术前准确评估。
Objective To study the index of surgical excision of primary reputured hepatocellular carcinoma.Methods 16 cases of sponta- neous rapture of hepatocellular carcinoma that received hepatectomies between Janunary 1993 and December 2003 were analysed retrospectively, and compared with 16 cases nonspontaneous reputure of hepatocellular carcinoma as control group at the same time.Results The diameter of livar tumor,intraoperative blood loss the postoperatiwe lncidence of complioation and hospital ization mortality rate of spontaneous reputure of hepato- cellular carcinoma were more than those of non-ones significantly,however preoperative child-puge grades of liver function,carcinoma cover, portal vein erosion,the portal clamping duration,operation duration and duration in hospital had no significant difference between the two groups.Conclution Whether spontaneous reputure of hepatocellular carcinomas were operated in emergency period lies in the accurate evaluation of preoperative indications.
