摘要
目的 :观察醒脑开窍活血法治疗高血压性脑出血的临床疗效。方法 :将 5 8例出血量≥ 30 ml的高血压性脑出血患者随机分为治疗组 30例和对照组 2 8例。两组均在常规治疗基础上进行早期颅内血肿微创清除术。治疗组加醒脑静注射液 2 0 ml静脉滴注 ;大黄粉每次 1g,每日 3次口服或鼻饲 ;脑出血方治疗。结果 :治疗组总有效率 (80 .0 % )和显效率 (6 6 .7% )与对照组 (6 4 .3%和 39.3% )相比差异均有显著性 (P均 <0 .0 1) ;治疗组日常生活能力显著优于对照组 (P <0 .0 5 ) ;治疗组平均催醒时间 (2 8.4 h)和再出血率 (7.7% )与对照组(40 .8h和 16 .7% )相比差异均具有显著性 (P均 <0 .0 1) ;治疗组的病死率稍低于对照组 ,但差异无显著性(P>0 .0 5 )。结论 :醒脑开窍活血法治疗高血压性脑出血能明显提高总有效率和患者的日常生活能力 ,缩短昏迷时间 ,减少再出血率。
Objective: To observe the clinically therapeutic effects of agents for restoring consciousness, opening the orifice and activating blood circulation (醒脑开窍活血法) on hyertensive cerebral hemorrhage(HCH).Methods: Fiftyeight cases with HCH (bleeding volume≥30 ml) were randomly divided into treatment group(30 cases) and control group(28 cases). Both groups were early treated by microinvasive hematoma removal operation of intracranial hematoma on the basis of routine treatment. Additionally, 20 ml Xingnaojing injection (醒脑静注射液) containing the above agents was injected intraveously and 1 g of rubarb powder dissolved in water was admininstered orally or via a gastric tube 3 times a day in the treatment group. Results: In the treatment group, the total effective rate( 80 0% vs. 64 3%) and the markedly effective rate(66 7% vs. 39 3%) were higher (both P <0 01), the ability of dailylife(ADL) was better( P <0 05), the average analeptic time was shorter(28 4 hours vs. 40 8 hours) and the rehemorrhagic rate(7 7% vs. 16 7%) were lower (both P <0 01) compared to the control group. The mortality was a little bit lower in the treatment group compared to that in the control group, but the difference was not significant ( P >0 05) . Conclusion : The treatment with methods of restoring consciousness, opening the orifice and activating blood circulation for hypertensive cerebral hemorrhage can increase the total effective rate significantly, improve the ADL, shorten the comatose time, and decrease the rehemorrhagic rate.
