Mapping with subjective and objective measures in cochlear implant users

摘要 目的 通过对人工耳蜗植入者术后调试中进行主观心理物理测试和电诱发听性脑干反应(ElectricallyEvoked Auditory Brainstem Responses(?)"EABR),神经反应遥测(Neural Response Telemetry,NRT)、电诱发镫骨肌反射(Electrically Evoked Stapedius Responses,ESR)等客观检测,探讨对人工耳蜗植入者协同应用主、客观方法进行术后调试的临床应用价值。方法对62名人工耳蜗植入者进行主观心理物理测试,测定主观阈值(T-levels)与主观最大舒适强度(C-levels),分别选取第3,10,20电极,进行EABR,NRT,ESR客观方法检测,测定阈值,将主观方法获得的T-levels与C-levels与EABR、NRT、ESR客观检测所测得的阈值进行比较。结果第3,10,20电极主观T-levels平均值分别为147.71±12.40,147.76±113.33,142.13±11.89电流级(current level,CL),C-levels平均值分别为203.26±14.43,203.32±15.85,198.51±15.16CL;EABR平均阈值分别为158.69±10.89,159.85±11.32,153.11±10.42CL;NRT检测ECAP平均阈值分别为191.56±12.15,193.69±8.80,184.35±9.35CL;ESR平均阈值分别为208.25±13.53,208.09±12.13,202.71±11.11CL。主观T-levels与EABR阈值间存在显著性相关(P<0,01);主观C-levels与ESR阈值间存在显著性相关(P<0.01)。结论1.在人? Objective Subjective psychophysical measurements, electrically evoked auditory brainstem responses (EABR), neural response telemetry (NRT) and electrically evoked stapedius responses (ESR) were performed in cochlear implant (Cl) users to discuss the feasibility and significance of mapping with subjective and objective measures. Methods Psychophysical measurements were performed in 62 cochlear implant users to get the subjective perceptual thresholds (T-levels) and maximum comfort levels (C-levels); EABR, NRT and ESR were recorded in electrodes No.3, 10, 20 of these 62 cochlear implant users to detect the objective thresholds. Then, for each electrode, correlation between the subjective and objective results was studied using Linear Regression analysis. Results Average T-levels in electrodes No.3, 10, 20 were 147.71 ± 12.40, 147.76 ± 113.33, 142.13 ± 11.89 current level (CL) respectively, average C-levels were 203.26 ± 14.43, 203.32 ± 15.85, 198.51 ± 15.16CL; average thresholds of EABR measured in electrodes No.3, 10, 20 were 158.69 ± 10.89, 59.85 ± 11.32, 153.11 ± 10.42 CL; average thresholds of ECAP were 191.56 ± 12.15, 193.69 ± 8.80, 184.35 ± 9.35CL; average thresholds of ESR were 208.25 ± 13.53, 208.09 ±12.13, 202.71 ± 11.11 CL; EABR thresholds were strongly correlated with T-levels (P0.01), ESR thresholds were strongly correlated with C-levels (P<0.01). Conclusions 1.When mapping after cochlear implant, subjective measurements combined with objective method such as EABR and ESR will make the programming process more accurate and efficient, especially for the patients who are not cooperative. 2. Objective measures are of great importance in the programming process of young children cochlear implant users.
