嗅鞘细胞移植改善多发性硬化患者的脊髓神经功能:1例报告(英文) 被引量:2

摘要 背景:多发性硬化一般应用激素、免疫抑制剂和干扰素等药物治疗,目前治疗效果均不够理想。目的:探讨嗅鞘细胞移植治疗脊髓多发性硬化的可行性,以及其改善多发性硬化脊髓神经功能的效果。设计:以诊断为依据前后对照、开放性实验研究。地点和对象:解放军海军总医院神经外科;1例多发性硬化女性患者,33岁,主诉四肢及躯干进行性无力,感觉减退2年。干预:取胚胎嗅球,消化成单个嗅鞘细胞后,培养2周,然后将其移植到脊髓病变部位。主要观察指标:进行术前和术后4周运动、轻触觉和痛觉功能评分对比。结果:嗅鞘细胞移植后4周,患者的脊髓功能有部分改善,美国脊髓损伤学会(ASIA)运动总分由术前2分增加到4分;轻触觉总分由4增到6分;痛觉由21增至24分。经电话随访半年,运动、感觉以及呼吸功能呈继续改善趋势。结论:嗅鞘细胞移植能改善多发性硬化患者的脊髓神经功能。长期效果有待进一步评价。 BACKGROUND:The treatment of multiple sclerosis(MS) includes prescription of steroid hormones,immunosuppressants and interferons.However,the curative effect on it is not satisfactory. OBJECTIVE:To explore the feasibility of olfactory ensheathing cell(OEC) transplantation in the treatment of MS and to evaluate the improvement of neurological function of spinal cord. DESIGN:An open and self-controlled trial based on diagnosis. SETTING and PARTICIPANT:The trial was conducted in Department of Neurosurgery,Navy General Hospital of Chinese PLA.One 33-year-old female patient who was diagnosed with MS had suffered from gradually progressive weakness and hypoesthesia for 2 years. INTERVENTION:Embryo olfactory bulb was obtained and trypsinized to single OECs,which were cultured for 2 weeks and then transplanted into the lesion sites of spinal cord. MAIN OUTCOME MEASURES:The spinal cord function including motor,light touch and pain sensation was assessed and compared before operation and 4 weeks after operation. RESULTS:The spinal cord function was improved partly 4 weeks after OEC transplantation.The American Spinal Injury Association(ASIA) motor score increased from 2 points pre-operation to 4 points post-operation;Light touch score from 4 to 6 points;and pain sensation score from 21 to 24 points.Her neurological function including motor,sensory and respiratory function continued to improve in the following half a year. CONCLUSION:OEC transplantation can improve the neurological function of spinal cord in the patient with MS.Further research into the long-term outcome of the treatment is required.
