摘要
目的:探讨基质金属蛋白酶(MMP)在骨肉瘤中的表达及与侵袭转移的关系。方法:应用免疫组化方法检测40例ⅡB期骨肉瘤MMP鄄7、MMP鄄9及MMP鄄10的表达,并分析它与无瘤生存率及侵袭转移的关系。结果:40例骨肉瘤MMP鄄7、MMP鄄9及MMP鄄10的阳性率分别为62.5%(25/40)、67.5%穴27/40雪和42.5%(17/40)。复发转移者MMP鄄7、MMP鄄9及MMP鄄10阳性率显著高于无瘤生存者(P<0.05);;累计无瘤生存率阳性组均低于阴性组(P<0.05),与接受辅助化疗患者分析结果类似。多因素分析提示,MMP鄄9表达是影响无瘤生存率的预后因素。结论:MMP鄄7、MMP鄄9及MMP鄄10在骨肉瘤中有不同程度的表达,与骨肉瘤细胞的高侵袭转移能力相关,其中MMP鄄9阳性表达有预后意义。
Objective: To investigate the expressions of matrix metalloproteinases(MMPs) in osteosarcomas and their relationship with local invasion and metastasis. Methods: Tumor biopsies from 40 cases of ⅡB osteosarcomas were chosen to study the expressions of MMP-7, MMP-9 and MMP-10 with immunohistochemistry, and their relationships with disease-free survival were analysed with Spearman, Kaplan-Meier methods and Cox regression model. Results: The expression rates of MMP-7, MMP-9 and MMP-10 were 62.5%(25/40), 67.5%(27/40) and 42.5%(17/40), respectively. Spearman test showed that patients with metastasis and recurrence had higher MMP-7, MMP-9 and MMP-10 expression rates than those with disease-free survival(P<0.05). Single variable analysis showed that the patients with positive expressions of MMP-7, MMP-9 or MMP-10 had significantly lower disease-free survival rates than those with negative staining (P<0.05). The analysis on patients who received adjuvant chemotherapy had the similar results. Multivariate analysis showed that only the expression of MMP-9 was an independent prognostic factor for disease-free survival. Conclusion: MMP-7, MMP-9 and MMP-10 are expressed in osteosarcoma cells and correlate with high invasive and metastatic tendency,but only MMP-9 correlates with prognosis significantly.
