摘要
目的 :比较削痂保留健康脂肪组织与切痂至深筋膜两种植皮方式对烧伤患者关节功能恢复的影响。方法 :抽取我院 1991～ 1997年收治的 5 8例 (组Ⅰ ) ,1998～ 2 0 0 3年收治的 6 3例 (组Ⅱ )涉及四肢关节深度烧伤并得到随访的病例 ,组Ⅰ切痂至深筋膜、组Ⅱ削痂保留健康脂肪组织 ,植皮、关节部位均应用大张中厚皮覆盖。结果 :两组间皮片成活率无统计学差异 ,但组Ⅱ患者四肢大关节及虎口功能畸形少、外形明显好于组Ⅰ。结论
Objective:In order to know if reserving adequate depth of healthy fat tissue at wound bed would improving the joint function of extremities postoperatively, a total of 121 patients with deep burns on joint sites in limbs, which were admitted to our hospital since 1991, were randomly selected for the study. Methods:58 cases (1991-1997) received split thickness autograft at the plane of deep fascia (group Ⅰ) while the other 63 cases (1998- 2003) reserved adequate depth of healthy fat tissue at wound bed (group Ⅱ). Results:The occurrence of dysfunction in major joints and fist finger webs was significant lower, and the external appearance was more satisfactory in group Ⅱthan groupⅠ. Conclusion:Reserving adequate depth of healthy fat tissue at wound bed on joint site is beneficial not only to improve limb function but also to increase life quality in burn patients.
