冠状动脉狭窄程度及稳定性与血浆可溶性细胞黏附分子的关系

The relationship between severity and stability of coronary atherosclerosis and soluble cell adhesion molecules

摘要 目的 检测冠心病 (CHD)患者血浆可溶性细胞黏附分子水平 ,探讨其与冠状动脉狭窄程度及稳定性的关系。方法 用ELISA法检测 6 0例经冠脉造影诊断的CHD患者血清sICAM - 1、sVCAM - 1、sE -selectin水平 ,并以 18例健康体检者作为对照。结果 与对照组比较 ,sICAM - 1水平CHD组增高 ,具有显著性意义 (P <0 .0 5 ) ,在急性冠脉综合征 (ACS)组增高更为显著 (P <0 .0 1) ;sVCAM - 1、sE -selectin水平在SAP和ACS两组间有显著性差异 (P <0 .0 1) ,而sICAM - 1水平在两组间无显著性差异 (P >0 .0 5 )。偏相关分析显示 ,sICAM - 1、sVCAM - 1与sE -selectin及Gensini积分无相关性。结论 CHD患者血浆sICAM - 1、sVCAM - 1、sE -selectin水平增高 ,3种可溶性细胞黏附分子水平均与冠状动脉狭窄程度无关 ,而sVCAM - 1、sE Objective To explore the relationship between soluble cell adhesion molecules and severity and stability of coronary atherosclerosis.Methods The level of sICAM-1, sVCAM-1 and sE-selectin in plasma measured for 60 patients with coronary heart disease diagnosed by coronary arteriography and 18 healthy persons as controls.Results The level of sICAM-1 with CHD was significantly higher than that of controls, especialy in ACS groups, but has no difference between SAP group and ACS group. The ACS group had higher level of sVCAM-1 and sE-selectin than that in SAP group(P<0.01). Multivariable analysis (stepwise regression) showed that Gensini's score were not associated with levels of sICAM-1, sVCAM-1 and sE-selectin.Conclution The level of sICAM-1, sVCAM-1 and sE-selectin were increased in patients with CHD. The stability of coronary atherosclerosis were associated with the level of sVCAM-1 and sE-selectin while the severity of coronary atherosclerosis were not associated with these three type of cellular adhesion molecules.
