摘要
目的 :探讨膝关节液透明质酸含量与关节软骨损伤程度之间的关系。方法 :采用ELISA法检测膝关节疾病患者 10 2例共 10 4膝的关节液透明质酸含量 ,并在关节镜下应用Outerbridge关节软骨损伤评分法和Ayral滑膜炎评分法评价膝关节软骨损伤程度和滑膜炎病理变化程度。采用t′检验、方差分析、Spearman相关分析和多元线性回归分析进行统计分析。结果 :临床诊断为OA组的关节液透明质酸含量较非OA组低 (t′ =- 2 186 ,P <0 0 5 )。Outerbridge软骨损伤累计评分≥ 10分组关节液透明质酸含量比Outerbridge软骨损伤累计评分 <10分组低 (t′ =- 2 316 ,P <0 0 5 )。 10 4膝关节液透明质酸含量除主要与Ayral滑膜炎评分呈正相关 (β′A=0 4 97,P <0 0 0 1)之外 ,还与Outerbridge软骨损伤累计评分呈负相关 (β′O=- 0 36 4 ,P <0 0 0 1)。在Ayral滑膜炎评分≥ 6 0分组 ,关节液透明质酸含量与Outerbridge软骨损伤累计评分呈负相关 (β′O=- 0 4 37,P <0 0 0 1) ,与Ayral滑膜炎评分呈正相关 (β′A=0 339,P <0 0 1) ,其中关节软骨累计损伤程度的影响较大。结论 :关节液透明质酸除主要反映膝关节滑膜炎程度外 ,在一定程度上还可反映关节软骨累计损伤程度。当滑膜炎较重时 ,后者则起主导作用。
Objective:[To explore the relationship of hyaluronic acid level in the synovial fluid of the knee with the degree of cartilage injury.Methods:[A total of 104 knees in 102 patients with knee osteoarthritis(OA)or other knee diseases were studied.The hyaluronic acid level in the synovial fluid of the knees was measured with enzyme linked immunoassay.The pathology of the synovium and articular cartilage was observed with arthroscopy,and degree of synovitis and cartilage injury under arthroscopy was graded with Ayrals score system for evaluation of the synovitis and Outerbridges score system for cartilage injury.The data were analyzed by t′ test,analysis of variance,Spearman correlation and multiple liner regression.Results:[The results showed that the hyaluronic acid level in the synovial fluid of OA knees was lower (t′=-2 186,P<0 05) than other knee diseases.The hyaluronic acid level in the synovial fluid of the knees was lower in those with accumulative Outerbridges score≥10 than in those with the score<10(t′=-2 316,P<0 05).The hyaluronic acid level in the synovial fluid of the knees was correlated positively with Ayrals score(β′ A=0 497,P<0 001) and negatively with accumulative Outerbridges score (β′ O=-0 364,P<0 001),especially Ayrals synovitis score in 104 knees.The level of hyaluronic acid in the synovial fluid in the knees with Ayrals score≥60 was correlated negatively with accumulative Outerbridges score (β′ O=-0 437,P<0 001) and positively with Ayrals score (β′ A=0 339,P<0 01),especially accumulative Outerbridges score.Conclusion:[The hyaluronic acid level in the synovial fluid of the knee can reflect the degree of synovitis and accumulative cartilage injury,especially synovitis.It reflects the degree of accumulativ
