摘要
目的研究超薄微型角膜刀的不同刃度对分离角膜上皮层的影响,来探讨制作角膜上皮瓣的合适刃度。方法采用旋转式微型角膜板层刀具,选用60 μm刃口厚度,随机选用不同刃度的角膜刀在新鲜离体人眼分离角膜上皮层,观察瓣的成形、手术床的平整情况,并作病理学检查。结果刃度在80~100 gf 之间的角膜刀片制作的角膜瓣成形好、瓣完整,病理检查提示为角膜上皮瓣,不含前弹力层,PAS 染色示基底膜完整。刃度≤70 gf 的角膜瓣成形好、瓣亦完整,病理检查示角膜瓣除角膜上皮层外,含有完整的前弹力层及少许角膜基质。刃度≥110 gf 的瓣成形差、为不完整瓣、手术床面不平整,病理检查示角膜上皮内的分离。当刃度≥140gf 时,不能进入角膜。结论刃度在80~100 gf 之间的超薄微型角膜刀可以用来制作角膜上皮瓣。
Purpose To study the effect of corneal epithelial separation with different blade edge sharpness(BES)of the ultra-thin mierokeratome and to investigate the suitable BES for the flap of corneal epithelium.Methods THe depth was 60 μm.The shaping of epithelial flap and sub-epithelial layer was observed and examined in histopathology.Results The corneal flap made with 80~100 gf BES of the microkeratome took good shape.Histopathological study showed the flap contained only epithelial layer without Bowman's membrane.And the basement membrane was intact.The corneal flap made with 70gf or under BES of the microkeratome also took good shape and was intact.Histopathological study showed it included not only epithelium,but also complete Bowman's membrane and a bit of corneal stroma.The corneal flap made with 110gf or mere edge degree of the microkeratome took bad shape and was incomplete.It was separated in the inner epithelium histopathologically.If the BES of the microkeratome was over 140 gf,it could not enter the cornea.Conclusion Corneal epithelial flap could be made with 80~100 gf BES degree of the ultra-thin microkeratome.
