摘要
目的 了解口服异维A酸治疗尖锐湿疣 (CA)的效果及其对复发的作用。方法 将门诊确诊的CA病人分为 2组 :异维A酸组 6 0例 ,给予异维A酸胶丸 10mg ,1日 3次或 1日 2次口服 ,同时用CO2 激光祛除疣体 ,每周随访 ,观察皮损变化及主要不良反应 ,并记录结果。将其中痊愈病人改为每日 1次 ,每次 10mg ,连续 1个月。停药再随访 3个月评定复发率。对照组 6 0例 ,α 干扰素 (沈阳三星生产 ) 30 0万U隔日 1次肌内注射 ,疗程及观察、记录方法同治疗组。结果 异维A酸组 :痊愈 2 4例 (4 0 0 % ) ,显效 35例 (5 8 3% ) ,无效 1例 (1 7% )。α 干扰素组 :痊愈 13例 (2 1 7% ) ,显效 4 4例 (73 3% ) ,无效 3例 (5 0 % )。两组治愈率比较差异有显著的统计学意义 (χ2 =3 76 ,P <0 0 5 )。采用口服异维A酸治疗痊愈的 2 4例病人中 ,复发 3例 ,复发率为 12 6 % ,采用肌内注射α 干扰素痊愈的 13例中 ,复发 7例 ,复发率为 5 3 8% ;异维A酸组近期复发率明显低于α 干扰素组。两组复发率的差异有显著的统计学意义 (χ2 =3 88,P <0 0 5 )。结论 口服异维A酸胶丸对CA具有较好的治疗作用 ,近期复发率较低。
Objective To understand the therapeutic effect of isotretinoin in condylomata acuminata(CA) and its recurrence rate.Methods After laser treatment of visible genital warts,60 cases with CA were treated with isotretinoin which was administered 30mg or 20mg every day until the end of treatment(complete warts clearance or up to a maximum of 6 weeks),and continued for l month,followed by a three-month treatment-free observation for the assesssment of its recurrence rate.The control group treated with α-interfen(3,000,000 unit im,once every two days) was observed and followed-up in the same way as the study group treated with isotretinoin.Results Of the 60 patients in the study group,24 were cured(24/60,40.0%),35 well responded (35/60,58.3%),and 1 had no response(1/60,1.7%).Of the 60 cases treated with α-interferon in the control group,13 were cured (13/60,21.7%),44 well responded (44/60,73.3%),and 3 had no response(3/60,5.0%).There were obvious statistic differences in the cure rate(χ2=3.76,P<0.05).Of the 24 patients with Isotretinoin,recurrence rate was 12.6%(3/24).Of the 60 cases treated with α-interferon in the control group,recurrence rate was 53.8%(7/13).There were obvious statistic differences in the recurrence rate(χ2=3.88,P<0.05).Conclusion The treatment with isotretinoin appears to be effective and and helpful for reducing the recurrence of genital warts.
