急诊首次确诊2型糖尿病伴亚临床动脉硬化患者的危险因素研究

Ultrasonic examination of subclinical atherosclerosis and its risk factors in the newly diagnosed type 2 diabetics in emergency department

摘要 目的 探讨急诊首次确诊2型糖尿病(2型DM)患者亚I临床动脉粥样硬化的发生情况、危险因素及患者的临床特点.方法 采用无创伤高分辨超声血管成像技术检查103例,急诊首次确诊2型糖尿病患者的颈总动脉,股动脉内中膜厚度(IMT)及动脉粥样硬化斑块.分别比较老年组与中青年组,老年组有无亚临床动脉粥样硬化患者在性别、体质量指数、血压、血脂、糖化血红蛋白水平及胰岛素抵抗等方面的差异.采用Logistic回归分析方法分析急诊首次确诊2型糖尿病患者亚临床动脉粥样硬化发生的危险因素.结果 老年亚临床动脉粥样硬化组8例(20.5%),中青年组11例(17.2%,P<0.01).股动脉亚临床动脉粥样硬化检出率高于颈总动脉分别为(9.2%、5.4%,P<0.01).收缩压水平:老年组(128±20)mm:Hg 1 mm Hg=0.133 kPa高于中青年组[(120±21)mm Hg,P<0.05];舒张压水平:老年组亚临床动脉粥样硬化(83±8)am Hg高于无动脉粥样硬化组[(76±7)mm Hg,P<0.01].Logistic回归分析显示亚临床动脉粥样硬化发生的危险因素为老龄化、胰岛素抵抗、舒张压水平增高及低水平的高密度脂蛋白胆固醇(p<0.05).结论 急诊首次确诊2型糖尿病部分存在亚临床动脉粥样硬化,老年患者患病率更高;不同部位亚临床动脉粥样硬化的发生率存在差异;急诊首次确诊2型糖尿病伴亚临床动脉粥样硬化的老年患者舒张压水平增高,其危险因素为老龄化、胰岛素抵抗、舒张压水平增高及低水平的高密度脂蛋白胆固醇.
