摘要
目的研究当完全门静脉动脉化(PVA)时,大鼠部分肝脏切除(肝切)后血清蛋白质及氨基酸谱水平的变化,来探讨PVA对肝脏蛋白质合成及氨基酸代谢的影响。方法采用袖套法建立大鼠PVA动物模型,每组随机选取SD大鼠20只,A组为PVA联合40%肝切+左肾切除组,B组为40%肝切+左肾切除组,C组为单纯左肾切除组。术后3天和7天用全自动氨基酸分析仪检测血清氨基酸谱水平。结果 A组手术成功率为93.3%(28/30),B组和C组手术成功率均为100%。血清氨基酸谱:Ⅰ:A、B两组大鼠于术后第3天血清中丙氨酸、络氨酸、苯丙氨酸、赖氨酸水平较C组明显升高,缬氨酸、亮氨酸、异亮氨酸、精氨酸、色氨酸水平及支链氨基酸/芳香族氨基酸(BCAA/AAA)比值较C组明显降低,差异均有统计学意义(均P<0.05),其中A组缬氨酸水平较B组明显降低,亮氨酸和赖氨酸水平较B组明显升高,差异均有统计学意义(均P<0.05),其余氨基酸水平比较差异均无统计学意义(均P>0.05)。Ⅱ:A组大鼠于术后第7天血清中异亮氨酸、亮氨酸、精氨酸、色氨酸水平及BCAA/AAA比值较B组明显升高,络氨酸、苯丙氨酸水平较B组明显降低,丙氨酸、赖氨酸水平较C组升高明显,差异均有统计学意义(均P<0.05)。B组大鼠血清中丙氨酸、络氨酸、苯丙氨酸、赖氨酸水平较C组明显升高,异亮氨酸、亮氨酸、精氨酸、色氨酸水平及BCAA/AAA比值较C组明显降低,差异均有统计学意义(均P<0.05),其余氨基酸水平比较差异均无统计学意义(均P>0.05)。结论 PVA后可有效地恢复血清氨基酸水平,且动脉化组较非动脉化组更能在短期内迅速恢复,对早期血浆氨基酸水平代谢调节有增强作用。
Objective To study the changes in the level of serum proteins and amino acids in rats performed partial hepatectomy when portal vein was fully arterialized,and to explore the effect on protein synthesis and amino acid metabolism in the liver by portal vein arterializations(PVA). Methods The rat model of PVA was established using cuff technique. Twenty of the Sprague-Dawley rats in each group were randomly selected. Rat models in group A were those with portal vein arterialization combining 40%liver resection and left nephrectomy,those in group B were with 40%hepatectomy with left nephrectomy,and those in group C were performed the left nephrectomy alone . On the third and seventh day after operation,the levels of serum amino acid profile were analyzed by automatic amino acid analyzer. Results The success rate of group A was 93.3%(28/30),and that of group B and C were both 100%. Serum amino acids:Ⅰ:3 days after surgery,Ala,Tyr,Phe,Lys in group A and B compared with group C increased highly;Val,Ile,Leu,Arg,Trp,BCAA/AAA in group A and B compared with group C decreased highly,all of the difference had statistically significant(all P<0.05). Comparing with group B,Val in group A decreased highly;Leu and Lys increased highly,with statistically significant difference(all P<0.05);while no statistically significant difference had been found in other parameters(all P>0.05).Ⅱ:7 days after surgery,Ile,Leu,Arg,Trp,BCAA/AAA in group A comparing with group B increased highly,Tyr,Phe decreased highly;when comparing with group C,Ala,Lys increased highly,and all of the difference was statistically significant(all P<0.05). Ala,Tyr,Phe, Lys in group B comparing with group C increased highly, and Ile,Leu,Arg,Trp,BCAA/AAA decreased highly with statistically significant difference(all P<0.05). As far as the changes of other amino acids were concerned, no statistically significant difference had been found between each other group(all P>0.05). Conclusions The levels of amino acids recover effectively after portal vein arterialization
