摘要
目的探讨肌钙蛋白I及肌钙蛋白T对于病毒性心肌炎的诊断和临床病情观察的价值。方法本研究选取2010年4月至2013年4月本院接收治疗的病毒性心肌炎患者112例作为观察组,选取同期内于本院参加健康体检的患者112例作为健康对照组,对患者入院时、入院后第1、2、3周、入院后3个月的肌钙蛋白I、肌钙蛋白T以及心肌酶谱进行含量测定。结果观察组患者肌钙蛋白I、肌钙蛋白T以及心肌酶谱各项指标含量均显著高于健康对照组(P<0.01),并且该检测方法不受患者性别的影响。在健康对照组患者中,肌钙蛋白假阳性率显著低于心肌酶谱假阳性率,证明通过肌钙蛋白诊断该疾病具有更高的准确率(P<0.05);患者在入院前2周肌钙蛋白有所下降,但不明显,从患者入院3周之后,肌钙蛋白量开始显著下降(P<0.05),而患者心肌酶谱各项测定值下降均无显著性差异(P>0.05)。结论肌钙蛋白I及肌钙蛋白T测定对于诊断病毒性心肌炎相对于心肌酶谱各项测定值具有更高的准确性,及时反映患者的病情变化,能够为治疗该疾病提供更为及时、可靠的指导。
Objective To investigate troponin I and troponin T for diagnosis of viral myocarditis and clinical disease observed value. Methods This study selected from April 2010 to April 2013 in our hospital to receive treatment for 112 patients with viral myocarditis as the observation group and then select done in our hospital during the same period of 112 patients healthy as a healthy control group, and patients admitted to hospital when 1, 2, 3 weeks after admission, three months after admission the patient's troponin I, troponin T and myocardial enzymes to determine the content. Results The patients troponin I, troponin T and myocardial enzymes indicators were significantly higher than the healthy control group(P < 0.01), and the detection method was not influenced by patient gender. In the healthy control group of patients, troponin false positive rate was significantly lower false positive rate of myocardial enzymes proved through troponin diagnose the disease has a higher accuracy rate(P < 0.05); patients in the hospital in the first 2 weeks, patients with troponin declined, but not obvious, from the hospital three weeks later, the patient began to significantly decrease the amount of troponin(P < 0.05), while the measured value decreases myocardial enzymes in patients with had significant difference(P > 0.05). Conclusion The determination of troponin I and troponin T for diagnosis of viral myocarditis with respect to the measured value of myocardial enzymes with higher accuracy and timely response to changes in the patient's disease, to be able to provide more timely treatment of the disease, reliably guiding value.
