子宫内膜癌组织中上皮型钙黏蛋白、神经型钙黏蛋白、雌激素受体、孕激素受体和P53的表达及意义 被引量:11

The expressions of E-cadherin,N-cadherin,ER,PR and P53 in endometrial carcinoma tissues and their clinical significance

摘要 目的观察上皮型钙黏蛋白(E-cadherin)、神经型钙黏蛋白(N-cadherin)、雌激素受体(ER)、孕激素受体(PR)、P53在子宫内膜癌组织中的表达水平,并探讨其临床意义。方法采用免疫组化法检测63例子宫内膜癌组织中E-cadherin、N-cadherin、ER、PR、P53的表达,分析上述指标的表达水平与临床病理特征的关系。结果 E-cadherin的阳性表达率在Ⅱ~Ⅲ期、深肌层浸润及有淋巴结转移组的子宫内膜癌组织中明显降低(P<0.05)。且E-cadherin的低表达与低分化(G3)有关。相反,N-cadherin在Ⅱ~Ⅲ期、G2~G3及有淋巴结转移组的阳性表达率明显升高(P<0.05);在非子宫内膜样腺癌、有附件转移及脉管瘤栓组表达率也有上升趋势。ER、PR的阳性表达率随着子宫内膜癌临床分期、病理分级的上升而逐渐降低,而P53的阳性表达率则升高。Bivariate相关性分析显示E-cadherin与ER、PR的表达呈正相关。结论 E-cadherin、ER、PR的表达下调和N-cadherin、P53的表达上调与子宫内膜癌的侵袭转移有关。 Objective To observe the expressions of E-cadherin, N-cadherin, ER, PR and P53 in endometrial carcinoma tissues and evaluate their clinical significance. Methods A total of 63 patients with endometrial carcinoma were enrolled into this study. The immunhistochemistry method was applied in detaching the level of E-cadherin, N-cadherin, ER, PR and P53. The relationship between the expression of these indicators and clinicalpathological features was evaluated. Results The positive expression rate of E-cadherin were significant lower in the patients with Ⅱ-Ⅲ stages,deeper myometrial invasion and lymph node metastasis(P<0.05). The decreased expression of E-cadherin was correlated with lower pathological grades(G3). On the contrary,the expression of N-cadherin increased in the patients with Ⅱ-Ⅲ stages, G2-G3 and lymph node metastasis(P<0.05), as well as in those patients with adenosquamous carcinoma,accessories metastasis and vascular tumor thrombus. As the stages and grades increased,the expression level of ER and PR in endometrial carcinoma tissue decreased, the level of P53 increased vice versa. The correlate analysis of Bivariate showed that the expression of E-cadherin had positive correlation with ER and PR. Conclusions The decreased expression level of E-cadherin,the increased expression level of ER,PR and the increased expression level of N-cadherin and P53 are significantly related with the invasion and metastasis in endometrial carcinoma.
