摘要
目的探讨子宫内膜癌宫外转移的影响因素。方法回顾性分析重庆市永荣矿业公司总医院2008年1月~2013年12月149例子宫内膜癌病例的临床资料。记录子宫内膜癌病例年龄、病理类型、绝经状况、肌层浸润深度、组织学分级、子宫内膜癌高危因素、雌激素受体（ER）、孕激素受体（PR）、肿瘤蛋白53（P53）、癌抗原125（CA125）表达情况等并分析其与子宫内膜癌宫外转移的关系。结果子宫内膜癌宫外转移与病理类型、组织学分级、肌层浸润深度、高危因素及ER、PR、CA125的表达状况密切相关（P〈0.05或P〈0.01）,而与年龄、绝经状况、P53的表达状况无显著关系（P〉0.05）;组织学分级3级（OR=5.367,95%CI：2.451~11.759）、肌层浸润深度≥1/2（OR=5.778,95%CI：1.963~17.009）、子宫内膜癌高危因素（OR=5.178,95%CI：3.963~16.009）、ER阴性（OR=5.358,95%CI：2.435~10.762）、PR阴性（OR=5.769,95%CI：1.971~17.217）、CA125阳性（OR=5.356,95%CI：2.439~10.772）等因素与子宫内膜癌宫外转移密切相关,为子宫内膜癌宫外转移的独立危险因素。结论子宫内膜癌的病理类型、组织学分级、肌层浸润深度、高危因素及ER、PR、CA125的表达状况是子宫内膜癌宫外转移的主要影响因素。
Objective To study influence factors of extrauterine metastasis in endometrial carcinoma. Methods The clinical data of 149 patients with endometrial carcinoma from January 2008 to December 2013 in General Hospital of Chongqing Yongrong Mining Company were retrospectively analyzed. Endometrial cancer pathological type, age, menopausal status, depth of myometrial invasion, histological grade, the high risk factors of endometrial cancer, and the expression of estrogen receptor （ER）, progesterone receptor （PR）, tumor protein 53 （P53）, cancer antigen 125 （CA125） were recorded, and the relationship between the these indexs and endometrial carcinoma extrauterine metastasis were analyzed. Results Endometrial cancer extrauterine metastasis is closely related to pathological type, histologic grade, depth of myometrial invasion, risk factors and ER, PR, CA125 expression state （P〈0.05 or P〈0.01）, and had no sig-nificant relationship with the P53 expression status and age, menopausal status （P〉 0.05）; histological grade level 3 （OR=5.367, 95%CI2.451-11.759）, myometrial invasion depth more than 1/2 （OR=5.778, 95%CI1.963-17.009）, the high risk factors of uterine endometrial carcinoma （OR=5.178, 95%CI3.963-16.009）, and negative for ER （OR=5.358, 95%CI2.435-10.762）, and negative for PR （OR=5.769, 95%CI1.971-17.217）, CA125 positive （OR=5.356, 95%CI2.439-10.772） were closely related with uterine endometrial carcinoma extrauterine metastasis, and were independent risk factors for ectopic endometrial metastasis endometrial cancer. Conclusion Endometrial cancer＇s pathological type, histologic grade, depth of myometrial invasion, risk factors and ER, PR and CA125 expression status are the main influ-ence factors of extrauterine metastasis of endometrial carcinoma.
