摘要
目的探讨超声与核素动态显像检查在诊断干燥综合征中的作用和价值。方法对98例干燥综合征(SS)和30例非干燥综合征患者分别行超声和核素动态显像检查;所有病例均经唇腺活检病理证实。结果SS组患者腮腺声像图表现较为特异,与对照组比较腮腺厚度、实质点状回声增粗率、实质回声不均匀率、实质回声减低率增加,以Ⅲ、Ⅳ级多见,共56例。彩色多普勒血流成像(CDFI)显示SS组收缩期峰值流速(PSV)明显高于对照组,阻力指数RI则下降(P<0.01)。超声诊断的敏感度为93%,特异度91%;核素动态显像的敏感度为86%,特异度92%,两组比较差异无统计学意义(P>0.05)。结论对临床疑诊为干燥综合征的患者应先行超声检查,若超声表现不典型可行唇腺活检确诊。
Objective To quantitatively evaluate ultrasonography and radionuclide imaging in the diagnosis of parotid gland involvement in Sjogren syndrome(SS). Methods Ultrasonography and radionuclide imaging were conducted on 98 cases with primary Sjogren syndrome according to new international criteria and on 30 normal subjects without the disease. Lip gland biopsies were done in all patients of the two groups for confirmating the two methods. Results The ultrasonographic appearance of the parotid in group SS was characteristic. The thickness, coarseness of bright spots and heterogeneous or low echoes in the parenchyma of the parotid gland were significantly higher in group SS than those in the normal group. Ⅲ and Ⅳ types were regularly met in 56 cases. In CDFI:PSV was significantly higher, RI was significantly lower in group SS than those in the normal (P<0.01). The sensitivity and specificity of ultrasonography were 93% and 91% respectively, while these of radionuclide imaging were 86% and 92% respectively. Two methods were of no significant difference statistically (P>0.05). Conclusions As for diagnosis of Sjogren syndrome, ultrasonography may be the first choice modelity and for atypical ones. Lip gland biopsiy showld be followed for confirming diagnosis.
