急性心肌梗塞患者心肌血流、脂肪酸代谢及糖代谢的变化及临床价值的探讨

摘要 目的 :探讨急性心肌梗塞患者心肌血流、脂肪酸代谢及糖代谢的变化及其临床价值。方法 :在 31例急性心肌梗塞患者中 ,进行 2 0 1 Tl、1 2 3 I- β- BMIPP SPECT及 1 8F- FDG PET心肌显像 ,将左心室分成 9个节段 ,采用 4级评分法对 2 0 1 Tl、1 2 3I- BMIPP分布进行视觉的评价 ,定性及定量测定 1 8F- FDG摄取。结果 :在血流灌注低下的 112个节段 ,血流 -代谢匹配的节段为 48个 ( 42 .9% ) ,不匹配的节段为 6 4个 ( 5 7.1% ) ;血流 -代谢不匹配节段的 92 .2 % ,匹配节段的 5 2 .1% (血流、代谢均严重降低者占 33.3% ) ,1 8F- FDG PET显示糖代谢亢进。结论 :在急性心肌梗塞 ,评价心肌血流、脂肪酸代谢及糖代谢的变化对判断存活心肌具有重要价值。 Objective:Our purposes were to observe the changes of myocardial perfusion, fatty acid metabolism, and glucose metabolism in patients with acute myocardial infarction, and to evaluate its clinical value.Methods:In 31 patients with acute myocardial infarction, we performed single emission computed tomography (SPECT) with 201 Tl, 123 I beta methyl iodophenyl pentadecanoic acid (BMIPP), and positron emission tomography (PET) with 18 F Fluorodeoxglucose (FDG) within one week.
