摘要
目的 探讨实时三维超声心动图 (RT 3DE)在房间隔缺损封堵术中的应用价值。方法 术前经二维超声心动图及RT 3DE检查明确诊断 ,术中全程引导临床 8例房间隔缺损 (ASD)介入治疗手术 ,术后经RT 3DE检查证实封堵效果。结果 8例介入手术均获成功 ,术后无残余分流、心律失常、心包填塞、封堵器脱落、栓塞等并发症。结论 RT 3DE可对缺损的部位、分型、直径、周长、面积做出准确定量诊断 ,无需经食管超声 (TEE)和球囊测径 ,可靠性强 ,且远离放射线。对封堵伞与缺损及心脏内部结构之间的精确定位及操作引导具有二维超声、经食管超声和放射线不可比拟的优势。
Objective To discuss the feasibility of RT-3DE(real time three-dimensional echocardiography) in independently guiding closure of ASD (Atrial septal defect). Methods The patients were diagnosed by RT-3DE before operation,the eight closure of ASD have been guided only by RT-3DE all throw the closure operation. The superior effect of the operation were proved by RT-3DE. Results All the operations were held successfully. The complications of remnant diffluence, arrhythmia, cardiac tamponade, embolism and the shedding of occluder were not observed. Conclusion RT-3DE can diagnose defect locus,type,diametes,circumference and area quantitively with accuracy. It can do without TEE or measuring sacculus diametes. It has quite good reliability. What's more, free from X-ray.It has unmatchable superiority of guiding operations and locating defect,occluder and the heart interior structure accurately compared with TEE and X-ray.
