摘要
目的探讨术中预置食管探条对腹腔镜胃底折叠术患者恢复及舒适度的影响。方法选取2017年9月至2019年6月于首都医科大学宣武医院确诊为胃食管反流病且需接受腹腔镜胃底折叠术(Nissen fundoplication)患者78例。根据随机数字表法,随机分为预置组和常规组,每组患者39例。常规组术中麻醉给予常规置管,预置组在术中麻醉后预置食管探条。比较2组患者的手术时间、术中出血量、并发症、住院时间及术后舒适度。结果预置组患者的舒适度评分(4.05分±1.21分)优于常规置管组(6.79分±1.42分),预置组手术时间(76.00 min±9.38 min)明显短于常规组(90.10 min±13.76 min),差异均有统计学意义(P<0.05)。预置组术后咽部疼痛发生率明显低于常规组,差异有统计学意义(P<0.05)。预置组患者住院时间明显低于常规组(5.72天±1.41天vs6.61天±1.84天),差异有统计学意义(P<0.05)。结论相对于常规组,预置组有效缩短患者手术时间、住院时间、降低咽喉疼痛发生率,且可提高患者术后舒适度,有利于患者康复。
ObjectiveTo explore the effect of intraoperative preset esophageal bougie on the recovery and comfort of patients undergoing laparoscopic fundoplication.MethodsA total of 78 patients who were diagnosed as gastroesophageal reflux disease and underwent laparoscopic fundoplication(Nissen fundoplication)in Xuanwu Hospital of Capital Medical University from September 2017 to June 2019 were selected and analyzed.According to the method of random number table,the patients were randomly divided into preset group(n=39)and routine group(n=39).The routine group was given routine catheterization during operation,and the preset group was given esophageal bougie after intraoperative anesthesia.The operation time,intraoperative blood loss,complications,hospital stay and postoperative comfort were compared between the two groups.ResultsThe comfort of patients in the preset group(4.05±1.21)was better than that in the routine group(6.79±1.42).The operation time in the preset group(76.00 min±9.38 min)was significantly shorter than that in the routine group(90.10 min±13.76 min),the incidence of postoperative pharyngeal pain in the preset group was significantly lower than that in the routine group,and the hospitalization time in the preset group was significantly lower than that in the routine group(5.72 day±1.41 dayvs6.61 day±1.84 day),which the difference was statistically significant(P<0.05).ConclusionCompared with the routine group,the preset group can effectively shorten the operation time,hospital stay and reduce the incidence of throat pain,and the intraoperative preset esophageal bougie can improve the postoperative comfort and facilitate the rehabilitation of patients.
