视知觉学习在青光眼患者视功能康复中的应用

The Application of Visual Perception Learning in the Rehabilitation of Visual Function in Glaucoma Patients

摘要 目的:探讨视知觉学习在青光眼患者视功能康复中的应用价值。方法:前瞻性病例对照研究。招募2018年4月至2022年6月在通用医疗成都三六三医院眼科就诊且病情控制稳定的青光眼患者43例(60眼),并将其随机分为对照组22例(30眼)和研究组21例(30眼)。对照组进行常规治疗,研究组在常规治疗的基础上,接受每日1次,为期3个月的视知觉学习,分别在学习前,学习后1周及1、3、6个月检测最佳矫正视力(BCVA)、眼压、视野平均缺损(MD)和视网膜神经纤维层(RNFL)厚度。对照组在没有任何其他干预的情况下维持常规治疗,并且在同样时间节点完成相同的视功能检查。采用重复测量方差分析,对2组治疗前和治疗后1周及1、3、6个月的BCVA、眼压、MD及RNFL厚度进行比较。结果:2组间在治疗前各指标间比较差异均无统计学意义(均P>0.05)。学习后研究组的各个阶段BCVA均高于对照组,进一步两两比较显示在治疗后1个月对照组和研究组BCVA分别为0.21±0.25和0.11±0.20,差异有统计学意义(P=0.049)。2组眼压、MD及RNFL厚度在各时间节点上差异均无统计学意义(均P>0.05)。结论:视知觉学习能有效改善青光眼患者BCVA,为青光眼的康复提供一种新的策略,并能进一步改善青光眼患者的生活质量。 Objective:To investigate the application value of visual perception learning in the rehabilitation of visual function among glaucoma patients.Methods:In this prospective case control study,a total of 43 glaucoma patients(60 eyes)with stable condition were recruited from department of Ophthalmology,GENERTEC 363 Hospital from April 2018 to June 2022.And participants in this study were randomly divided into a control group of 22 people(30 eyes)and an experimental group of 21 people(30 eyes).The control group received routine treatment.In addition to undergoing routine treatment,the experimental group received visual perception learning once a day for 3 months.The best corrected visual acuity(BCVA),intraocular pressure,mean defect(MD),and mean retinal nerve fiber layer(RNFL)thickness were measured before and 1 week,1 month,3 months,and 6 months after learning.The control group maintained routine treatment without any other intervention and completed the same visual function examination at the same time point.The BCVA,intraocular pressure,MD of the visual field,and RNFL thickness value were compared between the two groups before training and at 1 week,1 month,3 months,and 6 months after training using repeated measures analysis of variance.Results:There was no statistical difference of indicators before treatment between the two groups(all P>0.05).At the first month after the learning,the BCVA of the two groups was 0.21±0.25 and 0.11±0.20,with statistically significant difference(P=0.049).However,the difference of BCVA between two groups at other stage after treatment had no significant difference.There were no significant differences in MD,intraocular pressure,and RNFL thickness at each time point(P>0.05).Conclusion:Visual perception learning can effectively improve BCVA in glaucoma patients,which providing a new strategy for the rehabilitation of glaucoma and further improving the quality of life of glaucoma patients.
