3D打印技术辅助下全髋关节置换术对复杂髋部疾病患者影像学指标及髋关节功能的影响

Effects of Total Hip Arthroplasty Assisted by 3D Printing Technology on Imaging Indicators and Hip Function in Patients with Complex Hip Diseases

摘要 目的探讨3D打印技术辅助下全髋关节置换术对复杂髋部疾病患者影像学指标及髋关节功能的影响.方法回顾性分析2019年1月—2021年10月我院收治的52例复杂髋部疾病患者的临床资料,将接受常规全髋关节置换术治疗的26例设为对照组,将接受3D打印技术辅助下全髋关节置换术治疗的26例设为观察组,均观察至术后3个月.比较两组的手术情况、影像学指标、髋关节功能及并发症情况.结果观察组的手术时间、下床活动时间及住院时间分别为(67.50±8.21)min、(2.19±0.40)d、(9.38±1.53)d,均短于对照组,观察组的出血量为(313.46±27.99)mL,少于对照组,组间差异有统计学意义(P<0.05).观察组的术后臼杯前倾角偏差、外展角偏差、髋关节旋转中心垂直与水平距离偏差分别为(2.55±0.30)°、(1.99±0.38)°、(9.48±1.28)mm、(6.23±1.17)mm,均低于对照组,组间差异有统计学意义(P<0.05).术后3个月,观察组的Harris髋关节功能评分为(84.12±7.09)分,高于对照组的(76.73±5.86)分,差异有统计学意义(P<0.05).两组的并发症发生率比较,差异无统计学意义(P>0.05).结论3D打印技术辅助下开展全髋关节置换术可减轻手术创伤,增强关节稳定性,加快关节功能恢复,安全可靠. Objective To investigate the effect of total hip arthroplasty assisted by 3D printing technology on imaging indicators and hip function in patients with complex hip diseases.Methods The clinical data of 52 patients with complex hip disease admitted to our hospital from January 2019 to October 2021 were retrospectively analyzed.26 patients who received conventional total hip arthroplasty were set as the control group,and 26 patients who received total hip arthroplasty assisted by 3D printing technology were set as the observation group.All patients were observed until 3 months after surgery.The operation conditions,imaging indicators,hip joint function and complications were compared between the two groups.Results The operation time,ambulation time and hospital stay of the observation group were(67.50±8.21)min,(2.19±0.40)d and(9.38±1.53)d,respectively,which were shorter than those of the control group,the blood loss of the observation group was(313.46±27.99)mL,which was less than that of the control group,the differences between the groups were statistically significant(P<0.05).The deviation of anteversion angle of acetabular cup,deviation of abduction angle,deviation of vertical and horizontal distance of hip joint rotation center in the observation group were(2.55±0.30)°,(1.99±0.38)°,(9.48±1.28)mm,(6.23±1.17)mm,respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).At 3 months after operation,the Harris hip function score of the observation group was(84.12±7.09)points,which was higher than(76.73±5.86)points of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Total hip arthroplasty assisted by 3D printing technology can reduce surgical trauma,enhance joint stability,and accelerate joint function recovery,which is safe and reliable.
