摘要
目的探讨背根神经节射频术(PRF)联合药物治疗在带状疱疹后遗神经痛(PHN)患者中的应用价值.方法选择2020年6月-2022年3月该院收治的PHN患者82例,依据随机数字表法分为两组,每组41例.对照组采用药物治疗,研究组在对照组基础上采用PRF,两组均治疗28 d.对比两组疼痛视觉模拟评分(VAS)、匹兹堡睡眠质量指数量表(PSQI)评分、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、白细胞介素(IL)-10、IL-1β水平及不良反应发生情况.结果治疗前,两组VAS、PSQI评分相比,组间差异无统计学意义(P>0.05);治疗后,研究组VAS、PSQI评分均低于对照组,组间差异有统计学意义(P<0.05).治疗前,两组各项炎症指标水平相比,组间差异无统计学意义(P>0.05);治疗后,研究组TNF-α、CRP、IL-10、IL-1β水平均低于对照组,组间差异有统计学意义(P<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论PRF联合药物治疗能够有效降低PHN患者机体TNF-α、IL-10、IL-1β水平,减轻炎症反应,缓解疼痛,提高眠质量,安全可行.
Objective To investigate the application value of dorsal root ganglion radiofrequency(PRF)combined with drug therapy in patients with postherpetic neuralgia(PHN).Methods A total of 82 PHN patients admitted to our hospital from June 2020 to March 2022 were selected and divided into two groups according to the random number table method,with 41 cases in each group.The control group was treated with drugs,and the study group was treated with PRF on the basis of the control group.Both groups were treated for 28 d.The visual analogue score(VAS)for pain,Pittsburgh sleep quality index scale(PSQI)score,tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),interleukin(IL)-10,IL-1βlevels and adverse reactions were compared between the two groups.Results Before treatment,there were no significant differences in VAS and PSQI scores between the two groups(P>0.05);after treatment,the VAS and PSQI scores of the study group were lower than those of the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of various inflammatory indexes between the two groups(P>0.05);after treatment,the levels of TNF-α,CRP,IL-10 and IL-1βin the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion PRF combined with drug therapy can effectively reduce the levels of TNF-α,IL-10,and IL-1βin patients with PHN,reduce inflammation,relieve pain,and improve sleep quality,which is safe and feasible.
