摘要
目的探究艾滋病并发肺结核病的多层螺旋CT表现特点.方法选择2013年4月—2020年4月确诊的92例艾滋病并发肺结核病患者,根据CD4+检出值的不同将其分为A组(200~500 mm3)、B组(100~200 mm3)、C组(<100 mm3),均给予多层螺旋CT扫描检测,并对其影像学特点进行分析.结果在三组患者的CT成像中,A组患者多表现为斑片影、条索影及胸腔积液等,B组患者多表现为斑片影、结节影及条索影,而C组患者多表现为结节影、淋巴结肿大等,且C组患者的结节影及淋巴结肿大占比明显高于其他两组(P<0.05);此外,C组患者的病变范围明显比其他两组更广,其在双肺及左、右肺各部位的分布情况均多于其他两组患者(P<0.05).结论不同病情阶段的艾滋病并发肺结核病患者,其CT影像学特征也存在一定的差异,临床可结合其CT影像学表现,为艾滋病合并肺结核患者提供及时有效的诊断指导.
Objective To explore the characteristics of multi-slice spiral CT in AIDS complicated with pulmonary tuber⁃culosis.Methods:Select 92 patients diagnosed with AIDS complicated with tuberculosis from April 2013 to April 2020,and divide them into group A(200-500mm3),group B(100-200mm3)according to the difference of CD4+detection val⁃ue Group C(<100 mm3)was given a multi-slice CT scan,and the imaging characteristics were compared and analyzed.Results:In the CT imaging of the three groups of patients,the patients in group A mostly showed patch shadow,stripe shadow and pleural effusion,etc.,the patients in group B mostly showed patch shadow,nodule shadow and stripe shadow,while C Most of the patients in the group showed nodular shadow and lymphadenopathy,and the proportion of nodular shadow and lymphadenopathy in group C was significantly higher than that in the other two groups(P<0.05).Among the CT imaging results of the patients,group C The lesions of the patients were significantly wider than those of the other two groups,and their distributions in the lungs,left and right lungs were significantly more than those of the other two groups(P<0.05).Conclusion There are some differences in CT imaging features of patients with AIDS complicated by pulmo⁃nary tuberculosis at different stages of disease.Clinical CT imaging can be combined with clinical manifestations to pro⁃vide timely and effective diagnosis guidance for patients with AIDS and pulmonary tuberculosis.
