摘要
目的探讨髓鞘黏连相关蛋白(Caspr)及载脂蛋白(APOE)蛋白对2型糖尿病合并阿尔茨海默病(AD)患者神经功能的影响,并研究其作用机制。方法选取我院2017年1月到2018年12月50例2型糖尿病合并AD患者作为研究对象,同时选取2型糖尿病患者无AD组50例作为糖尿病组,选取50例AD组而无2型糖尿病患者作为AD组;选取同期在我院体检的健康者50例作为对照组。收集一般临床资料并采用PCR法检测Caspr mRNA、APOE mRNA、LC3Ⅰ、LC3ⅠmRNA水平并进行比较;评估4组患者入组后第1、14天NIHSS评分,采用MRS量表评估4组患者3个月后神经功能;采用Logistic回归分析NIHSS评分降低值、MRS评分与一般临床资料、Caspr mRNA和APOE mRNA之间的相关性。将Caspr、APOE质粒以及对应空载体PCDF质粒分别转导进入HEK293细胞,分为Caspr组、APOE组、空载组,利用Western blot检测Caspr、APOE和LC3(LC3Ⅱ/LC3Ⅰ)蛋白表达并比较。结果4组患者入组后第1天、第14天、随访3个月的Caspr mRNA、APOE mRNA、LC3Ⅰ、LC3Ⅰm RNA水平经统计学检验,差异均有统计学意义,其中对照组水平最高,AD组次之,糖尿病合并AD组最低(P<0.05);第1天、第14天的NIHSS评分以及随访3个月的MRS评分差异均有统计学意义(P<0.05)。Logistic回归分析结果表明,NIHSS评分降低值、MRS评分均与Caspr mRNA和APOE mRNA呈负相关(P<0.05)。Caspr组、APOE组LC3Ⅰ、LC3ⅠmRNA、LC3Ⅱ/LC3Ⅰ水平分别明显低于空载组(P<0.05)。结论Caspr蛋白和APOE蛋白可能是2型糖尿病合并阿尔茨海默病性痴呆神经功能损害的影响因素,并参与神经认知功能损害。
Objective To investigate the effects of myelin adhesion associated protein(Caspr)and APOE protein on the neurological function of patients with type 2 diabetes mellitus complicated with Alzheimer′s disease,and to investigate the mechanism of its neurological impairment.Methods 50 patients with type 2 diabetes combined with Alzheimer′s disease in our hospital from January 2017 to December 2018 were selected as research objects.Meanwhile,50 patients wit htype 2 diabetes were selected as diabetes group,50 patients with Alzheimer′s disease as AD group.50 healthy patients during the same period were selected as the control group.General clinical data were collected and the levels of Caspr mRNA,APOE mRNA,LC3Ⅰ,LC3ⅠmRNA were detected and compared by PCR.The level of Caspr,APOE and LC3(LC3Ⅱ/LC3Ⅰ)were determined and compared using Western blot detection.The NIHSS scores of the four groups on 1 d and 14 d after enrollment were evaluated,while the neurological functions of the four groups after treatment 3 m were evaluated by MRS scale.Logistic regression was used to analyze the relationship between the reduced NIHSS score,MRS score andgeneral clinical data,Caspr mRNA,APOE mRNA.Results There was significant difference for the level of Caspr mRNA,APOE mRNA,LC3Ⅰand LC3ⅠmRNA on 1 d,14 d,3 m among the four groups.The level of Caspr mRNA and APOE mRNA of control group was the highest,while the Alzheimer′s disease group was middle,the type 2 diabetes combined with Alzheimer′s disease group was the lowest(P<0.05).The MRS score on 1 d,14 d,3 m among the four groups showed significant difference(P<0.05).There were negative relationship between Caspr mRNA,APOE mRNA and NIHSS score,MRS score analyzed by Logistic regression analysis(P<0.05).The level of LC3Ⅰ,LC3ⅠmRNA and LC3Ⅱ/LC3Ⅰof Caspr group and APOE group were significantly lower than the light group,respectively(P<0.05).Conclusion Caspr protein and APOE protein are the influencing factors of neurological impairment in type 2 diabetes mellitus patients
