摘要
目的:系统评价心外膜脂肪组织(EAT)对冠心病的诊断价值。方法:检索PubMed、Embase、Web of Science、CNKI和CBM从建库到2019年1月关于EAT在冠心病中的临床应用试验。采用改进后的Jadad量表对文献进行质量评价,使用RevMan5.3软件对纳入的文献进行Meta分析。结果:最终纳入21篇文章,共4049例患者。超声心动图显示,冠心病组最大EAT厚度为(12.21±2.62)mm,最小为(2.22±1.86)mm;非冠心病组最大EAT厚度为(9.92±1.37)mm,最小为(1.8±1.4)mm。所纳入文献存在较大异质性(I^2=95%,P<0.05),采用随机效应模型进行分析,结果显示,冠心病组患者EAT厚度明显高于非冠心病组患者(SMD=1.07,95%CI:0.75~1.38,P<0.05);冠心病组患者超声心动图与CT测量的EAT厚度值均高于非冠心病组患者。结论:EAT具有临床易用性、成本效益和非暴露特征,超声心动图或CT测量EAT厚度应用于冠心病诊断及风险分层上有一定价值,值得进一步探索。
Objective:To systematically evaluate the diagnostic value of epicardial adipose tissue(EAT)in coronary heart disease.Method:Articles about clinical application trials of EAT in coronary heart disease were searched from PubMed,Embase,Web of Science CNKI and CBM from the establishment time to January 2019.The improved Jadad scale was used for quality evaluation,and Revman5.3 software was used for Meta-analysis.Result:Twenty-one articles including 4049 cases were retrieved.Echocardiography showed that the maximum and the minimum of EAT thickness in CHD group were(12.21±2.62)mm and(2.22±1.86)mm respectively,while those in non-CHD group were(9.92±1.37)mm and(1.8±1.4)mm.The random effect model was used for the analysis because of the great heterogeneity in literatures(I^2=95%,P<0.05).The results showed that the EAT thickness in CHD group was significantly higher than that in non-CHD group(SMD=1.07,95%CI:0.75-1.38,P<0.05),and the EAT thickness measured by echocardiography or CT in CHD group were higher than those in non-CHD group.Conclusion:Eat has characteristics of clinical ease of use,cost-effectiveness and non-exposure.EAT thickness measured by echocardiography or CT has a certain value in the diagnosis and risk stratification of coronary heart disease,which is worth further exploration.
