摘要
基于CHARLS 2018年数据,运用Heckman模型,研究家庭照料与老年人不同形式医疗服务利用之间的关系,并探究二者之间可能的影响路径。研究结果表明:家庭照料有可能通过提高老年人医疗服务可及性来释放其医疗需求,从而和医疗费用支出、就医次数呈现出显著的互补效应;通过中介效应模型对其中一个路径进行验证发现,接受家庭照料可以通过就医时长来影响老年人医疗服务费用。对策建议:对照料资源和医疗资源进行有效整合,以应对我国快速老龄化背景下医疗资源紧张的问题。
Using data from China Health and Retirement Longitudinal Study(CHARLS)in 2018,this paper studied the relationship between family care and utilization of various medical service for the elderly on the basis of the Heckman model,and explored possible influencing paths between the two.Results show that informal care can release medical demands by improving accessibility of medical service,thus showing a significant complementary effect with care expenses and the number of medical visits.Then,one of these pathways was validated through a mediation effect model,and it was found that receiving informal care could also affect medical service expenses of older adults by seeking medical treatment through the length of medical visits.Then it is proposed that we should integrate care and medical resources to alleviate medical resource constraints under the background of rapid aging in China.
