八段锦结合放松功训练对轻中度抑郁症沉思状态的调节 被引量:3

Modulation of Rumination Response for Mild to Moderate Depression by Traditional Chinese Medicine Relaxing and Calming Exercise

摘要 目的比较八段锦结合放松功训练(简称中医松静功)与常规肌肉放松训练对轻中度抑郁症情绪及自我倾注模式的调节作用。方法采用随机对照试验,选取2017年1月—2019年12月的抑郁症患者50例作为研究对象,随机分为两组,分别接受中医松静功与常规肌肉放松训练,干预6周。评估工具为沉思反应量表(RRS)、自动思维量表(ATQ)、正念注意觉知量表(MAAS)、心智游移频率问卷(MWFQ)、汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)、贝克抑郁量表(BDI),在治疗前与治疗6周后测评,实验组共有23例患者完成试验,对照组共有21例患者完成试验。结果与对照组相比较,发现中医松静功组的抑郁、焦虑水平在干预后较对照组显著降低(P<0.01);中医松静功组的沉思反应抑郁得分在干预后较对照组显著降低(P<0.01),强迫及反思得分、心智游移频率量表得分较对照组表现出降低趋势,正念注意觉知得分实验组较对照组表现出提高,但差异未达到统计学意义(P>0.05);组内比较,发现中医松静功组的自动思维、沉思反应3个成分、心智游移频率6周后显著降低,正念注意觉知增加(P<0.01或P<0.05)。结论中医松静功对改善焦虑情绪与改变抑郁症自我倾注状态更具有优势,尤其是能够改变沉思反应的抑郁成分、反思成分及强迫成分,还可以提高自我觉知注意功能,减少心智游移。 Objective To compare the regulatory effects of traditional Chinese medicine relaxing and calming exercise and conventional muscle relaxation training on mood and rumination mode of mild to moderate depression.Methods Fifty patients with depression from January 2017 to December 2019 were selected and randomly divided into two groups which were treated with traditional Chinese medicine relaxing and calming exercise and conventional muscle relaxation training respectively.The intervention period lasted for 6 weeks.The assessment tools were the Rumination Response Scale(RRS),Automatic Thinking Questionnaire(ATQ),Mindful Attention Awareness Scale(MAAS),Mind Wandering Frequency Questionnaire(MWFQ),Hamilton Depression Rating(HAMD),Hamilton Anxiety Rating Scale(HAMA)and Beck Depression Scale(BDI).Twenty-three patients in the experimental group completed the test,while there were 21 patients in the control group.Results Compared with those of the control group,the depression and anxiety levels in the traditional Chinese medicine relaxing and calming exercise group were significantly lower than those in the control group after intervention(P<0.01).The RRS-depression score of the traditional Chinese medicine relaxing and calming exercise group was significantly lower than that of the control group(P<0.01).Compared with those of the control group,the scores of RRS-compulsion and RRS-reflection showed a downward trend,but the difference was not statistically significant(P>0.05).In the intra-group comparison,the scores of automatic thinking,RRS-depression,RRS-compulsion and RRS-reflection in the traditional Chinese medicine relaxing and calming exercise group were significantly decreased after 6 weeks(P<0.01 or P<0.05).Conclusion The traditional Chinese medicine relaxing and calming exercise has more advantages in improving anxiety and modulating the state of self-focus,especially can change the depressive,reflective and compulsive components of rumination,and also can improve mindfulness state and reduce mind-wandering.
