摘要
【目的】探讨利多卡因对大鼠蛛网膜下腔出血后海马CA1区神经元的保护作用。【方法】将60只成年大鼠随机分为4组:正常组(6只)、假手术组(18只)、自发性蛛网膜下腔出血(subarachnoid hemorrhage,SAH)组(18只)、利多卡因组(18只);采用枕大池二次注血法制备动物模型。与第二次注射血液30 min后假手术组和SAH组注射生理盐水,利多卡因组注射盐酸利多卡因,在不同的时间段灌注后取脑,观察细胞形态、凋亡相关因子的改变。【结果】海马CA1区神经元HE染色可见SAH组神经细胞结构松散,网格形状改变,绝大多数的细胞破裂,大量的核溶解,而第二次注入血液后3、5、7 d破碎的细胞和细胞核溶解的数量逐渐增加。利多卡因组可见神经细胞核破碎成碎片,但相比SAH组数量少,周围细胞形状清晰,注射利多卡因后7 d与3、5 d比较不正常的细胞明显减少。免疫组化显示SAH组(3 d)开始B-淋巴细胞瘤-2基因(B-cell lymphoma-2,Bcl-2)相关X蛋白(Bcl2-associated X protein,Bax)表达增强,5、7 d的表达均为高表达;Bcl-2表达从3 d开始表达逐渐减低(P<0.01)。利多卡因组(3 d)开始Bcl-2表达增强,5、7 d的表达均为高表达;Bax表达从3 d开始表达逐渐减低。电镜显示SAH组和利多卡因组在不同时间段神经核和线粒体发生明显变化,而SAH组与利多卡因组相比较,在各时间段细胞核和线粒体的损伤都很严重,尤其在第7天。【结论】利多卡因可减轻大鼠SAH后海马CA1区细胞损伤,具有明显脑保护作用。
【Objective】To explore the protective effect of lidocaine on neurons in hippocampus CA1 after subarachnoid hemorrhage(SAH)in rats.【Methods】A total of 60 adult rats were randomly divided into 4 groups:normal group(6 rats),sham surgery group(18 rats),SAH group(18 rats)and lidocaine group(18 rats).The model was prepared by secondary blood injection in the occipital cistern.Rats in the sham surgery group and SAH group were given normal saline and lidocaine 30 minutes after the second injection.Brain perfusion was performed at different times to observe the changes of cell morphology and apoptosis-related factors.【Results】HE staining of neurons in hippocampus CA1 showed that the structure of nerve cells in the SAH group was very loose and presented sieves.Most of the cells were ruptured and a large number of nuclei were dissolved,and the number of ruptured cells and nucleolysis gradually increased on the 3 rd,5 th and 7 th days after the second blood injection.The nucleus in the lidocaine group was broken into fragments,but the number was small compared with that in the SAH group.The cell morphology of the surrounding area was basically distinguishable,and the number of abnormal cells was significantly reduced on the7 th day after the injection of lidocaine compared with those on the3 rd and 5 th days.Immunohistochemistry results showed that Bax expression was increased in the SAH group(3 d),and highly expressed on 5 d and 7 d.The expression of Bcl-2 decreased gradually from day 3(P<0.01).The expression of Bcl-2 in the lidocaine group(3 d)was enhanced,and the expressionon 5 d and 7 d was high.The expression of Bax decreased gradually from 3 d.The results of electron microscopy observation showed that the nucleus and mitochondria in the SAH group and lidocaine group were significantly changed in each phase,and the damage of nucleus and mitochondria was more serious in the SAH group than that in the lidocaine group,especially on 7 d.【Conclusion】Lidocaine can reduce the injury of neurons in the hippocampus
