摘要
多模态交际中最佳关联的寻求失败是医患沟通中情感误解的主因之一。基于关联理论,构建多模态语用视阈下医患情感误解分析框架,运用会话分析的研究方法对电视剧《急诊科医生》三类有代表性的医患沟通片段中的情感误解现象进行多模态语用解读。研究结果表明,情感误解主要存在于信息、预设和结论三个层面,若不及时处理,则会随着话轮推进加深误会,进一步诱发或激化医患冲突。情感误解的三个层面并非完全独立,彼此间存在交融现象,因而一次医患沟通中的情感误解可能会涉及其中的一个或多个层面。医患关系实为一种信托关系,规避医患间的情感误解需要交际双方尽可能在语言维度满足对方信息需求,在情感维度关注对方情感诉求,以平和的心态寻求双方话语中的最佳关联,方能积极推进医疗进程,构建和谐医患关系。
Doctor-patient communication is essentially multimodal,and the failure to seek the optimal relevance becomes an important reason leading to emotive misunderstanding.Based on relevance theory,this study aims to construct a multimodal pragmatic analytical framework and employ conversation analysis as a research method to explain the emotive misunderstanding in ER Doctors(a teleplay)by applying multimodal transcription and pragmatic interpretation.The study has shown that emotive misunderstanding can arise at the three levels of information,presupposition and implicated conclusions.Moreover,if the misunderstanding is not to be timely or properly handled,it will deepen and further intensify the conflict between doctors and patients with the development of the turn-taking.The three levels of emotive misunderstanding are not completely independent,and they blend with each other.Therefore,one or more of the above levels may be involved in a doctor-patient communication.Doctor-patient relationship is actually a kind of trust relationship.To avoid doctor-patient conflict and emotive misunderstanding,both sides should try their best to meet each other’s needs for information in language dimension,and pay attention to each other’s emotive demands in emotive dimension,and seek the optimal relevance in the interactive conversation with a peaceful attitude,so as to promote actively the medical treatment process and build a harmonious doctor-patient relationship.
