摘要
目的研究毛细支气管炎患儿血清中基质金属蛋白酶-9(MMP-9)、基质金属蛋白酶抑制因子-1(TIMP-1)和白细胞介素-17(IL-17)水平及其与反复喘息发生的关系。方法选取2018-09-01-2019-03-31郑州儿童医院收治的62例符合毛细支气管炎诊断标准的患儿作为研究对象,运用酶联免疫吸附法(ELISA)检测其血清MMP-9、TIMP-1和IL-17水平。出院患儿随访1年,根据喘息发作次数分为对照组(喘息发作<3次)及病例组(喘息发作≥3次)。结果56例患儿完成随访,其中病例组14例,对照组42例。病例组血清MMP-9含量为(262.677±57.999)ng/mL,高于对照组的(209.587±43.681)ng/mL,差异有统计学意义(t=3.620,P=0.001);病例组血清TIMP-1含量为(171.216±39.077)ng/mL,高于对照组的(141.252±26.485)ng/mL,差异有统计学意义(t=3.236,P=0.001);病例组血清IL-17含量为(84.590±14.123)pg/mL,高于对照组的(70.174±10.994)pg/mL,差异有统计学意义(t=3.951,P=0.001)。多因素分析结果显示,MMP-9(OR=0.618,95%CI为0.256~1.490)、TIMP-1(OR=1.175,95%CI为0.658~2.098)及IL-17(OR=4.790,95%CI为0.578~39.685)水平与喘息反复发作的关系无统计学意义,均P>0.05。结论反复喘息婴幼儿MMP-9、TIMP-1和IL-17水平有增高趋势,监测MMP-9、TIMP-1及IL-17水平可能对喘息反复发作的早期预测有一定积极作用。
Objective The purpose of this paper was to explore the relation between serum levels of matrix metalloproteinases-9(MMP-9),tissue inhibitors of matrix metalloproteinase-1(TIMP-1),interleukin-17(IL-17)and recurrence of wheezing episodes in children with bronchiolitis.Methods Totally 62 cases of children with bronchiolitis in Zhengzhou Children’s Hospital from September 1,2018 to March 31,2019 were collected as the clinical research objects.The concentration of MMP-9,TIMP-1,IL-17 in serum were determined by ELISA.The patients were followed for a year.According to the numbers of wheezing attacks,they were divided into the control group(<3 wheezing attacks)and the case group(≥3 wheezing attacks).Results Fifty-six children completed follow-up,including 14 cases in the case group and 42 cases in the control group.The serum MMP-9 content of the case group was(262.677±57.999)ng/ml,which was higher than the control group(209.587±43.681)ng/ml,and the difference was statistically significant(t=3.620,P=0.001).The serum content of TIMP-1 of the case group was(171.216±39.077)ng/ml,which was higher than that of the control group(141.252±26.485)ng/ml,the difference was statistically significant(t=3.236,P=0.001).The serum IL-17 content of the case group was(84.590±14.123)pg/ml,higher than the control group(70.174±10.994)pg/ml,and the difference was statistically significant(t=3.951,P=0.001).Multivariate analysis showed that there was no statistically significant relationship between the levels of MMP-9(OR=0.618,95%CI:0.256-1.490),TIMP-1(OR=1.175,95%CI:0.658-2.098)and IL-17(OR=4.790,95%CI:0.578-39.685)and recurrent wheezing.Conclusion The serum levels of MMP-9,TIMP-1,IL-17 in infants with recurrent wheezing show an increasing trend,and monitoring serum levels of MMP-9,TIMP-1,IL-17 may play apositive role in early prediction of recurrent episodes of wheezing.
