摘要
目的通过对2015—2019年儿科住院患者疾病谱分析,了解患儿疾病谱构成及变化规律,为儿童常见病的防治提供依据与对策。方法按照ICD-10对2015—2019年27618份儿科住院病例主要诊断进行疾病分类,描述性统计分析。结果①近5年南充市某三甲医院儿科出院人数逐年增加,男性患儿均高于女性患儿。②5年来呼吸系统疾病、消化系统疾病、传染病和寄生虫病一直位列儿科患者疾病谱前三顺位,以呼吸系统疾病居首,并且各年呼吸系统疾病均占儿科出院患儿的70%以上,其构成比最高为2019年79.02%,呼吸系统疾病中肺炎为首要疾病。消化系统疾病、传染病和寄生虫病、血液及造血器官和免疫机制疾病、皮肤和皮下组织疾病均位列2~5位,2015—2016年损伤中毒居第6顺位,2017—2019年降至第7顺位。结论南充市某三甲医院5年来儿科出院患儿呼吸系统疾病一直居疾病谱首位,应是儿童疾病重点防控方向。其次是消化系统疾病及传染病和寄生虫病,损伤中毒疾病亦不可忽视,加强健康宣教,落实到家庭、学校及社区有利于减少儿童患病风险。
Objective Through the analysis of the disease spectrum of pediatric hospitalized patients from 2015 to 2019,to understand the composition and change of the disease spectrum of children,and to provide basis and countermeasures for the prevention and treatment of common diseases in children.Methods According to ICD-10,the main diagnoses of 27618 pediatric hospitalized cases from 2015 to 2019 were classified and analyzed by descriptive statistics.Results 1.In the past five years,the number of pediatric discharges from a third-class hospital in Nanchong City has been increasing year by year,and male patients were higher than female patients.2.In the past five years,respiratory diseases,digestive diseases,infectious diseases and parasitic diseases had occupied the top three positions in the spectrum of pediatric patients,with respiratory diseases taking the lead,and respiratory diseases accounted for 70%of pediatric discharged patients in each year,the highest proportion was 79.02%in 2019,and pneumonia was the primary disease among respiratory diseases.Digestive system diseases,infectious diseases and parasitic diseases,blood and hematopoietic organs and immune mechanism diseases,skin and subcutaneous tissue diseases were all ranked 2-5,2015-2016 injury and poisoning ranked 6th,and 2017-2019 decreased to the 7th overall pick.Conclusion Respiratory diseases of discharged pediatric patients in a tertiary A hospital in Nanchong have been occupying the first place in the disease spectrum in the past five years,and should be the focus of prevention and control of childhood diseases.Second are diseases of the digestive system and infectious and parasitic diseases.Injury and poisoning diseases cannot be ignored.Strengthening health education and implementing them in families,schools and communities will help reduce children's risk of illness.
