摘要
目的对脂联素在妊娠期肝内胆汁淤积症中的定量检测和临床意义进行探讨。方法采集2018年4月1日-2019年9月30日医院分娩的孕妇60例,妊娠期肝内胆汁淤积症的30例为观察组,正常妊娠孕妇的30例为对照组。所有孕妇均接受酶联免疫吸附法检测脂联素水平,再以全自动生化仪检测总胆汁酸水平,分析总胆汁酸水与脂联素水平的相关性,探讨两种指标异常对于妊娠结局的影响。结果相较于对照组,观察组总胆汁酸水平更高,脂联素水平更低(P<0.05);脂联素水平降低与总胆汁酸水平升高具有负相关性(r=-0.523,P<0.05);相较于对照组,观察组不良妊娠结局发生率更高,剖宫产率更高(P<0.05)。结论妊娠期肝内胆汁淤积症患者中,脂联素水平与总胆汁酸水平呈现相关性;通过孕期脂联素检测能够有效预测妊娠期肝内胆汁淤积症,可使不良妊娠结局得到改善。
Objective To explore the quantitative detection and clinical significance of adiponectin in intrahepatic cholestasis of pregnancy.Methods 60 pregnant women delivered in hospital from April 1 st,2018 to September 30 th,2019 were collected,30 cases of intrahepatic cholestasis of pregnancy were the observation group(patients),and 30 cases of normal pregnant women were the control group.All pregnant women were tested for adiponectin level by ELISA,and then the total bile acid level was detected by automatic biochemical analyzer.The correlation between total bile acid water and adiponectin level was analyzed,and the effect of abnormal two indicators on pregnancy outcome was discussed.Results Compared with the control group,the total bile acid level in the observation group was higher and the adiponectin level was lower(P<0.05);the decrease of adiponectin level was negatively correlated with the increase of total bile acid level(r=-0.523,P<0.05);compared with the control group,the incidence of adverse pregnancy outcomes in the observation group was higher and the cesarean section rate was higher(P<0.05).Conclusions Adiponectin level was correlated with total bile acid level in patients with intrahepatic cholestasis of pregnancy.Pre-pregnancy adiponectin detection could effectively predict intrahepatic cholestasis of pregnancy and improve adverse pregnancy outcomes.
