摘要
目的:探讨脊柱受累的SAPHO综合征(滑膜炎-痤疮-脓疱病-骨质增生-骨炎)患者临床特点。方法:回顾性分析2013年1月至2020年10月住院诊治的48例SAPHO综合征患者的临床资料,根据脊柱受累与否分为脊柱型和非脊柱型,分析两组患者的一般特点、临床表现、影像学表现、实验室及病理学检查结果。结果:48例SAPHO综合征患者中脊柱型32例(66.7%)、非脊柱型16例(33.3%);与非脊柱型相比,脊柱型患者年龄偏大[(54.5±10.8)vs.(48.6±10.3)岁,P<0.05]、更多出现椎体部位疼痛(84.4%vs.18.8%,P=0.001)、C反应蛋白较高[9.8(3.2,20.5)mg/L vs.3.1(1.2,7.6)mg/L,P=0.024]。腰椎受累概率最高,其次为胸椎、颈椎(26.3%、19.3%、3.1%,P<0.01)。影像学检查提示患者总受累椎体数为123个,连续型共90个,胸椎病变椎体数最多,其次为腰椎、颈椎(74、42、7个,P<0.01)。结论:脊柱型SAPHO综合征常见,此类患者年龄偏大,多出现病变椎体部位疼痛,炎症反应更强烈,影像学检查可发现椎体病变多分布于胸腰椎。
Objective:To investigate the clinical characteristics of SAPHO syndrome patients with spinal involvement.Methods:The clinical data of 48 patients with SAPHO syndrome hospitalized from January 2013 to October 2020 were retrospectively analyzed.All patients were divided into two groups as spinal group and non-spinal group based on the presence of spinal lesions.The general characteristics,clinical manifestations,imaging findings,laboratory and pathological findings of the two groups were analyzed.Results:Among 48 patients with SAPHO syndrome,32(66.7%)were spinal type and 16(33.3%)were non-spinal type.The patients in spinal group were more older[(54.5±10.8)vs.(48.6±10.3)years old,P<0.05],there were more patients with pain in the vertebral bodies(84.4%vs.18.8%,P=0.001),and higher CRP[9.8(3.2,20.5)mg/L vs.3.1(1.2,7.6)mg/L,P=0.024]compared with the non-spinal group.The lumbar spine was most frequently involved,followed by the thoracic and cervical spine(26.3%,19.3%,3.1%;P<0.01).The imaging suggested that the total number of affected vertebral bodies was 123,among which 90 were continuous type.Thoracic spine lesions were the most,followed by lumbar spine and cervical spine(74,42,7;P<0.01).Conclusions:Spinal type SAPHO syndrome is common in older people with pain in the vertebral bodies and intense inflammatory reaction,and lesions are often located in the thoracolumbar spine.
