摘要 目的探讨以我院为首的医院集团前置审方系统的建立及其在合理应用中的应用。方法在HIS系统上嵌入前置审方系统,在公司提供原有规则库的基础上,根据实际情况制定出符合我院的审方用药知识库,并培养专职的审方药师。通过分别抽取2018年7~12月门诊处方作为对照组(前置审方开展前),2019年7~12月门诊处方作为观察组(前置审方开展后),比较前置审方系统运行前后门诊处方的合格率、不合理处方类型占比、不合理处方药师干预成功率、抗菌药物的使用率、住院抗菌药物的使用强度、药品不良反应的发生率等。结果对照组处方合格率为91.85%,观察组处方合格率为99.18%;前置审方开展后各类型处方的不合理率大大降低;前置审方开展前后,不合理处方干预成功率分别为25.3%、85.6%;前置审方开展前后,门诊抗菌药物的使用率平均分别为16.21%、9.32%,住院抗菌药物的使用率平均分别为55.50%、44.86%,住院抗菌药物使用强度平均分别为44.99%、39.64%;β-内酰胺类抗菌药物、中药注射剂不良反应,前置审方开展前、后这两类药品的不良反应发生率分别为18.26%、4.5%。结论药师借助前置审方系统进行处方审核,与传统的人工审核相比,能更及时发现潜在的不合理用药问题,弥补了事后点评处方用药情况滞后性,保障患者的用药安全。 Objective To explore the establishment of pre-trial system of hospital group led by our hospital and its application in rational application.Methods Embed pre reviewer system.Based on the original rule base provided by the company,a prescription medication knowledge base in line with our hospital is formulated according to the actual situation.Train full-time prescription reviewers.The outpatient prescriptions from July to December 2018 were selected as the control group(before the pre-trial party was carried out),and the outpatient prescriptions from July to December 2019 were selected as the observation group(after the pre-trial party was carried out).The qualification rate of outpatient prescriptions,the proportion of unreasonable prescription types,the success rate of intervention by unreasonable prescription pharmacists,the use rate of antibiotics,the use intensity of inpatient antibiotics and the incidence of adverse drug reactions were compared before and after the operation of the pre-trial prescription system.Results The qualified rate of the prescription in the control group was 91.85%,and that in the observation group was 99.18%;After the pre-trial,the unreasonable rate of various types of prescriptions is greatly reduced;Before and after the pre trial,the success rates of unreasonable prescription intervention were 25.3%and 85.6%respectively;Before and after the pre-trial,the average use rates of outpatient antibiotics were 16.21%and 9.32%,the average use rates of inpatient antibiotics were 55.50%and 44.86%,and the average use intensity of inpatient antibiotics were 44.99 and 39.64 respectively;β-The incidence of adverse reactions of lactam antibiotics and traditional Chinese medicine injections before and after the pre examination were 18.26%and 4.5%respectively.Conclusion Pharmacists conduct prescription review with the help of the pre review system.Compared with the traditional manual review,they can find potential unreasonable drug use problems in a more timely manner,make up for the lag of post
