CT与MRI诊断腹部侵袭性纤维瘤病的影像学表现分析 被引量:4

摘要 目的将CT与MRI用于腹部侵袭性纤维瘤病诊断中,对其影像学表现进行分析。方法选取2018年1-12月收治的50例经病理证实的腹部侵袭性纤维瘤病患者,所有患者均行CT、MRI诊断,对两种诊断方式影像学表现进行分析。结果 CT对AF检出率为90.00%,与MRI检出率92.00%相比,差异无统计学意义(P>0.05);该组50例AF单发46例,两个或以上4例,肿瘤直径4~11 cm,平均(7.56±0.25)cm,肿瘤部位:腹腔29例,腹膜后31例,肿块膨胀性呈圆形或类圆形31例,肿块浸润性生长成不规则形19例;AF的CT影像学表现主要为病灶大部分边界不清楚,CT可见尖角征及晕日征,病灶多为低或等密度,增强扫描可见病灶多为均匀、中度渐进性强化;AF患者的MRI影像学表现主要为病灶边界不清楚,MRI可见尖角征及晕日征,病灶大部分信号均匀(88.00%),T1WI呈低、等信号,T2WI呈略高、高信号,DWI大部分呈高信号,MRI增强呈均匀明显强化,强化过程与CT扫描一致,大部分为渐进性强化(94.00%),所有病变均未见钙化区、坏死及囊变。结论 AF在CT及MRI上具有特殊影像学特征,可为临床诊断提供重要依据。 Objective To use CT and MRI in the diagnosis of abdominal aggressive fibromatosis and analyze its imaging findings.Meth-ods A total of 50 patients with invasive abdominal fibromatosis confirmed by pathology were selected from January to December 2018.All patients were diagnosed with CT and MRI,and the imaging findings of the two diagnostic methods were analyzed.Results The CT detection rate of AF was 90.00%,which was not statistically significant difference compared with the MRI detection rate of 92.00%(P>0.05).In this group,50 cases of AF had single-onset 46 cases,two or more 4 cases,and 4-11 cm of tumor diameter,average(7.56±0.25)cm,tumor location:29 cases of abdominal cavity,31 cases of retroperitoneum,31 cases of swelling masses were round or round-like,19 cases of masses infiltrating and growing into irregular shapes;CT of AF The imaging findings are mainly that the boundaries of most of the lesions are not clear.CT showed sharp angle signs and halo signs.The lesions were mostly low or iso-den-sity.The enhanced scans showed that the lesions were mostly uniform and moderate progressive enhancement;MRI imaging of AF pa-tients of the main manifestations were that the boundary of the lesion was not clear,MRI can show sharp angle sign and halo sign,most of the lesion had uniform signal(88.00%),T1 WI was low and isointensity,T2 WI was slightly higher and hyperintensity,DWI was mostly hyperintensity.MRI enhancement showed uniform and obvious enhancement,and the enhancement process was consistent with CT scan,most of which were progressive enhancement(94.00%).There were no calcified areas,necrosis,and cystic changes in all le-sions.Conclusion AF has special imaging characteristics on CT and MRI,which can provide important basis for clinical diagnosis.
