摘要
目的分析比较CT与MRI对骨骼肌筋膜侵袭性纤维瘤病的评估价值。方法选取我院2015年2月至2017年3月收治的骨骼肌筋膜侵袭性纤维瘤病患者16例,并经手术病理学证实。9例患者于术前行CT检查(CT组), 10例患者行MRI检查(MRI组)。结果 (1)CT组密度改变:3例呈等密度, 6例呈高密度; 6例为病变密度, 1例可见边缘带状出血, 1例可见散在分布的不规则斑片状低密度病变, 1例可见条索状稍低密度病变; 1例病灶边界清晰, 8例边界模糊不清。(2)MRI组信号改变:T1WI平扫7例呈等信号, 3例呈低信号; T2WI平扫10例呈高信号; 4例可见斑点、斑片、条带样低信号病变散在分布; 6例沿肌间筋膜生长呈不规则状, 4例呈长梭形改变; 2例病灶局限于单一肌肉, 8例累及多块肌肉; 3例肿瘤边界清晰、可见不完整包膜, 7例边界模糊、无包膜,边缘可见爪状生长。结论与CT相比, MRI更能清晰显示骨骼肌筋膜侵袭性纤维瘤病患者病灶的形态和范围,影像学表现更具特征性。
Objective To analyze and compare the evaluation value of CT and MRI in aggressive fibromatosis of skeletal muscle fascia. Methods 16 cases of patients with aggressive fibromatosis of skeletal muscle fascia admitted to our hospital from February 2015 to March 2017 were selected and confirmed by surgery and pathology. 9 cases of patients received CT examination before operation（CT group）, and 10 cases of patients received MRI examinations（MRI group）. Results（1）The CT density change： 3 cases had ISO density, and 6 cases had high density; 6 cases were lesion density, 1 case showed marginal banded hemorrhage, 1 case showed scattered irregular patch low density lesions, and 1 case had streak slightly low density lesions. 1 case had clear boundary of the lesions, and 8 cases had blurred boundary.（2）MRI signal changes： T1 WI plain scan in 7 cases had equal signal, and 3 cases had low signal; T2 WI plain scan in 10 cases had high signal;4 cases had scattered spots, patches and stripe like low signal lesions; 6 cases had irregular growth along the intermuscular fascia, and 4 cases showed long spindle changes; 2 cases of lesions were confined to single muscle, and 8 cases involved multiple muscles; 3 cases had clear boundary of tumor and incomplete capsule, and 7 cases had blurred boundary and no capsule, with claw like growth on the edge.Conclusions Compared with CT, MRI can clearly show the shape and scope of lesions in patients with aggressive fibromatosis of skeletal muscle fascia, and the imaging features are more characteristic.
