摘要
目的探讨布氏杆菌脊柱炎的CT、MRI表现。方法回顾性分析6例布氏杆菌脊柱炎的CT、MRI表现,包括病变椎体密度及信号改变,椎间盘信号,椎间隙改变,有无死骨、椎旁脓肿等。结果CT显示6例椎体骨质破坏呈小类圆形或斑片样低密度灶,病灶见边缘硬化带,2例硬化带可见新破坏灶;MRI显示4例椎体破坏呈长T1长T2混杂信号影,2例椎体T1WI、T2WI呈低信号;3例椎间盘T1WI、T2WI呈低信号改变,3例椎间盘呈不均匀长T1长T2信号影。6例椎间隙均变窄;均未见死骨;椎小关节均未见破坏;4例见椎旁脓肿形成。结论布氏杆菌脊柱炎的CT、MRI表现具有一定的特点,影像学表现相邻椎体破坏伴骨质增生硬化,椎间隙变窄,可有椎旁脓肿或硬膜外脓肿,两者结合能为布氏杆菌脊柱炎的诊断提供影像学依据,提高对本病的认识,避免误诊,具有很高临床价值。
Objective To investigate the CT and MRI findings of brucella spondylitis.Methods CT and MRI findings of 6 patients with brucella spondylitis were retrospectively analyzed,including the density and signal changes of the diseased vertebral body,intervertebral disc signals,intervertebral space changes,dead bone and paraspinal abscess,etc.Results CT showed that 6 cases of vertebral bone destruction presented small round or patchy low-density foci,the lesions showed marginal sclerosis belt,and 2 cases showed new lesions in the sclerosis belt.Magnetic resonance imaging(MRI)showed that 4 patients showed long T1 and long T2 mixed signals in vertebral destruction,while 2 patients showed low signals in T1WI and T2WI.3 cases of intervertebral disc T1WI and T2WI showed low signal changes,and 3 cases of intervertebral disc showed uneven long T1 and long T2 signal shadows.The intervertebral space narrowed in all 6 cases.No dead bones were found;The facet joints were not damaged.Paravertebral abscess formation was seen in 4 cases.Conclusion Brinell coli spondylitis performances of CT,MRI have certain characteristics,exhibiting adjacent vertebral body destruction associated with bone hyperplasia on hardening,narrow intervertebral disc,but have a job by abscess or epidural abscess.A combination of these can provide imaging for the diagnosis of brinell coli spondylitis can improve the understanding of the disease,avoid misdiagnosis,which has high clinical value.
