Effects of Repeated Transcranial Magnetic Stimulation Combined With Mirror Therapy on Limb Function in Patients With Post-Stroke Hemiplegia

摘要 目的探讨治疗脑卒中后偏瘫采用重复经颅磁刺激(rTMS)结合镜像疗法对患者肢体功能的影响。方法选择2017年6月—2019年5月的90例脑卒中后偏瘫患者进行观察,随机分为两组,各45例,对照组接受常规康复治疗,观察组在对照组基础上加r TMS结合镜像疗法治疗,对两组治疗前后患侧脑区运动诱发电位皮质潜伏期(cortical latency,CL)、中枢运动传导时间(centralmotorconductivetime,CMCT)、Fugl-Meyer运动评定量表(fugl meyer exercise assessment scale,FMA)评分、日常生活活动能力评分(activities of daily living,ADL)进行观察。结果两组治疗前的CL及CMCT差异无统计学意义(P>0.05),观察组治疗后CL及CMCT优于对照组(P <0.05);治疗前两组FMA评分差异无统计学意义(P> 0.05),治疗后观察组均较对照组各评分更高(P <0.05);治疗前两组ADL评分差异无统计学意义(P> 0.05),治疗后观察组较对照组ADL评分更高(P <0.05)。结论 rTMS结合镜像疗法可改善脑卒中后偏瘫患者患侧大脑皮质兴奋性,提高肢体功能,也利于提高日常生活能力。 Objective To investigate the effects of repeated transcranial magnetic stimulation combined with mirror therapy on limb function in patients with hemiplegia after stroke.Methods 90 patients with post-stroke hemiplegia who were treated from June 2017 to May 2019 were selected as the research object,and the random number table method was used to divide the control group and the observation group into 45 cases.The control group received conventional rehabilitation treatment,and the observation group added repeat transcranial magnetic stimulation combined with mirror-image therapy on the basis of cortical latency(CL),central motor conduction time(CMCT),fugl-meyer exercise rating scale(FMA),upper and lower extremity scores,and daily life activity scores(ADL)of the affected brain area before and after treatment Observed.Results There were no significant difference in CL and CMCT before treatment between the two groups(P>0.05).CL and CMCT were significantly better in the observation group after treatment than in the control group(P<0.05);The FMA scores of the upper and lower extremities before treatment were not significantly different between the two groups(P>0.05),and the FMA scores of the upper and lower extremities of the observation group after treatment were higher than those of the control group(P<0.05);There was no significant difference in ADL scores between the two groups before treatment(P>0.05),and the ADL scores in the observation group were significantly higher than those in the control group(P<0.05).Conclusion Repeated transcranial magnetic stimulation combined with mirror therapy can excite the cerebral cortex on the affected side of patients with hemiplegia after stroke,improve limb function,and also improve daily living ability.
