摘要
目的探讨时差成像系统(TLI)挑选优质胚胎对体外授精-胚胎移植(IVF-ET)妊娠结局的影响。方法收集本中心2015年8月~2017年5月接受IVF-ET的不孕症患者98例,随机分组,采用TLI动态观察筛查胚胎进行ET的47例常规IVF治疗患者为实验组(A组),同期51例采用传统培养方法的常规IVF治疗患者为对照组(B组),根据形态学标准挑选胚胎进行ET,并比较妊娠结局。此外,再选取53例常规IVF治疗的患者共478枚胚胎,每个周期胚胎随机分为两组,采用TLI集簇培养的胚胎记为C组,微滴单胚胎常规方法培养记为D组,比较两组2PN来源的胚胎培养结果差异。结果实验组(A组)与对照组(B组)的D3优质胚胎率、临床妊娠率、胚胎种植率、早期流产率差异均无统计学意义(P> 0.05);实验组(A组)种植胚胎与未种植胚胎相比,原核消失时间(tPNF),发育至2细胞时间(t2)、2细胞分裂至3细胞持续时间(cc2)、3细胞分裂至4细胞持续时间(s2),差异有统计学意义(P <0.05),发育至3细胞、4细胞、5细胞时间(t3、t4、t5),差异均无统计学意义(P> 0.05)。C组与D组来源于2PN的胚胎,卵裂率、D3优质胚胎率、D3可利用胚胎率、囊胚形成率、优质囊胚率差异均无统计学意义(P> 0.05)。结论采用TLI挑选优质胚胎安全可行,非侵入性评估能够掌握完整的胚胎发育信息,TLI筛选参数可预测胚胎发育潜能,利于临床选择优质胚胎移植,改善妊娠结局。
Objective To investigate the effect of in vitro fertilization-embryo transfer pregnancy outcome by selecting high quality embryos with the time-lapse imaging system. Methods Ninety-eight patients with infertility who underwent IVF-ET treatment in our IVF center from August 2015 to May 2017 were selected, and all patients were divided into group A and group B. TLI was used to screen embryos for ET in 47 patients with conventional IVF treatment(group A), and 51 patients with conventional IVF treatment by traditional culture method in the same period were used as control group(group B). According to morphological criteria, embryos were selected for ET and pregnancy outcome were compared. In addition, a total of 478 embryos for 53 patients with routine IVF were selected at the same time, and the embryos at each cycle were randomly divided into two groups. Embryos cultured in TLI clusters were recorded as group C, and those cultured in micro-droplet routine single embryos were recorded as group D. The difference of embryo culture results from 2PN sources were compared. Results There was no significant difference in D3 high quality embryo rate, clinical pregnancy rate, embryo implantation rate and early abortion rate between group A and group B(P 〉0.05). The tPNF, 2-cell development(t2), 2-cell division to 3-cell duration(cc2) and 3-cell division to 4-cell duration(s2) of implanted embryos were significantly different from those of non-implanted embryos in group A(P 〈0.05).There was no significant difference in the t3, t4, t5(P 〉0.05).There was no significant difference from 2PN sources between group C and group D in the cleavage rate, D3 high quality embryo rate, D3 embryo usability rate, blastocyst formation rate and high quality blastocyst rate(P 〉0.05). Conclusion For embryo selection, TLI non-invasive assessment is feasible and safe. The time-lapse imaging parameters can predict the embryo developmental potency.
